- Some items, including markers in the preschool room, ink stamp sets, and face and body paints in the baby room, were observed without ACMI or ASTM safety certification marks. The operator was instructed to ensure that only approved, child-safe materials are used in the daycare.

- The illness incident log sheet had not been completed by staff since April 23, 2025. The operator was instructed to ensure that the log is filled out whenever a child becomes ill to support effective tracking and tracing of potential outbreak cases within the facility.

- The diapering pad in the toddler room was found to be ripped, compromising its ability to be properly sanitized. The operator was instructed to replace it with a new one.

- In the toddler and baby room, the diapering pad was placed directly on a flat surface without any protection from fall hazard. The operator was advised to install a surrounding lip or edge around the diapering pad to mitigate the risk of tripping hazards.

- Toys were observed in the hand washing sink located next to the diapering station. The operator was instructed to use this sink for hand washing purposes only.

- Light was observed coming through gaps underneath the exterior back door and the main entrance front door. The operator was instructed to install a strip to seal these gaps and mitigate the risk of pest activity.

- The area rug in the toddler room was observed to be visibly soiled.

- Potty training seats in the washroom, shared between the toddler and preschool rooms, were observed to have visible debris and dirt. The operator was instructed to ensure the seats are cleaned and sanitized properly and at frequent intervals.
