      • Dino Room:The handwashing sink located in the room was observed to be used for washing toys. Staff were advised to use the adjacent laundry room sink for cleaning and sanitizing toys.Ensure that the handwashing sink is used exclusively for hand hygiene purposes at all times to prevent cross-contamination.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • A plastic plant pot was observed placed on the cushioned surface of a playhouse cube, elevated at some height from the ground. The cushion surface was unstable, creating a risk that the pot could fall and potentially cause injury to children using the play area.Onsite staff promptly removed the pot and relocated it outdoors onto a firm, stable surface.To prevent physical hazards and reduce the risk of injury, do not store any items on soft or unstable surfaces.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • A tray of food (properly covered/ set out for the lunch period) was observed sitting out on the handwashing sink near diapering station of the Dino Kids room.Onsite staff acknowledged the issue and immediately removed the food tray from the area.Ensure not to store any food items in or around the handwashing station and the diapering station.Ensure the handwashing sink is always accessible/unobstructed for use.
    • 15. Is laundry being properly handled in a suitable manner?
      • A basket full of diapers was observed stored on the washroom floor of Bumble Forest room. Onsite staff indicated that those were unused additional set of diapers. In the laundry room, laundry baskets were not labeled. Lable the laundry basket and ensure that there is proper separation of cleaned and dirty laundry.Properly stored the unused/new diaper supplies on cleaned shelves/baskets above the ground.

      • 1. Mattress on the diapering table in Bumble Forest room was torn from the edge.Repair/replace the diapering mattress to ensure the diapering surface is easy to clean and disinfect.2. Absorbent couches and cushions were observed in the Dino Kids room.Use washable/cleanable covers on the cushion and couches OR remove them from the use.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. Dust builds up was observed on some of the toys in the Caterpillar Corner room and the Dino Kids room.Ensure the toys are cleaned and sanitized properly.2. Water table in the Caterpillar Corner room was observed wet with small amount of water pooling and debris in it.Ensure water play tables are kept clean, free from debris, and in good condition at all times (clean-disinfect and air dry when not in use).3. In Bumble Forest room, the play structures had a slide with a climbable height of 21 inches and stairs with a height of 31 inches. A carpeted floor was used as the surface beneath the structure. Those play structures were blocked, and the children did not have any access at the time of inspection.Install a fall protective surfacing to support the play slide or remove the structures.

      • Common for all blankets were stored in a single basket in the Caterpillar Corner room.Discussed with the onsite staff to store each blankets separately in a zip-lock bags and properly label them before storing in the same basket. Ensure that there is proper separation of personal belongings for all the kids.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Dust builds up was observed on the bathroom exhaust vent cover.Clean the vent cover on the regular basis for effective ventilation.

      • The chairs in the gym rooms were made of fabric material (absorbent), some of the chairs had stain, spillage marks.Ensure the indicated seating surfaces can be easily cleaned/laundered.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • No space for storage of personal belongings such as bags, water bottles, jackets, shoes etc.

      • -Handwashing sinks were not present in adjacent to the diapering stations. Action taken:-Concern discussed with the manager.-Instructed to move the diaper tables near to the handwashing sinks. AHS diapering steps poster was shared already with the manager.

      • -Handwashing sinks were not present in adjacent to the diapering stations. Action taken:-Concern discussed with the manager.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • -Absorbent couches and cushions were observed in the daycare. Action taken:-Since the couches and cushions were fibrous/made of absorbent material, they are not easily cleanable. Options provided to manager either to put the washable/cleanable covers on the cushion and couches OR remove them from the daycare.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • -The sand tables and water tables in the daycare had dirt/debris/sand- they were not maintained in cleaned condition. Action required:-Please clean and sanitize the water/sand tables after each use.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • -The bed linens for each client were not labeled. Manager confirmed that staff is aware of the personal linen for each client. Action required:-Labelling of the bed linens are required.
    • 28. Are washrooms / bathing facilities properly constructed, maintained and supplied (Critical)?
      • -The washroom near to the kitchen area was not maintained in the cleaned and sanitary condition.-Bad smell was coming out. Action taken:-The issue has been discussed with the manager.

    • 06. Are proper protocols being followed in regards to client care?
      • -Inappropriate diapering procedure was observed. Following observations were made1. The diapering station was not nearby to the handwashing station for easy accessibility to wash hands during diapering.2. Staff member did not wash her hands and child's hand after diaper change.3. No availability to disinfectant (bleach) on some diaper stations. 4. No availability of the closed lid plastic lined garbage bins for used diapers. Actions taken:-The concern was discussed with the manager. AHS diapering procedure protocol was provided via email. Manager was instructed to train to follow appropriate diapering steps. -Staff was instructed to place the diapering stage nearby to the handwashing station as possible.
