      • Fail to provide at least 14 days written notice to the health department before commencing operation of intent to operate, name and location of intended premises, operator name and contact information, and list of personal services provided.     • Work surfaces, reusable items and/or equipment contaminated with blood and/or body fluid cleaned and disinfected as soon as possible while wearing appropriate personal protective equipment (PPE)
      • Fail to provide high-level disinfectant for disinfecting reusable items and/or equipment contaminated with blood and/or body fluid. Operator/PSW must provide high-level disinfectant for disinfecting reusable items and/or equipment contaminated with blood and/or body fluid.
    • All reusable items and/or equipment cleaned and then disinfected or sterilized as applicable
      • Fail to disinfect reusable items and/or equipment after each use with intermediate-level disinfectant (e.g. tweezers, comedones/extractors). Operator/PSW must disinfect reusable items and/or equipment after each use with intermediate-level disinfectant (e.g. tweezers, comedones/extractors).
