- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- (1) Operator completed the following written procedures at the time of inspection: (a) Accidental Skin Cuts or Punctures (b) Cleansing Skin (c) Applying Antiseptic.* Permanent Makeup Pre-Appointment, Healing Process, & Aftercare information is provided on the business website. Clients are given a care package and the link to the site. This is an acceptable form of the required written procedures for post-service care.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- (1) Sterility assurance documentation was not being kept. Information that needs to be documented includes: (a) Lot number that matches the sterility assurance documentation, (b) Manufacturer or company name and contact information. When this info is only found on the box/container keep it for your records. (c) Method of sterilization. (d) Expiry date.* Operator is keeping digital records of general client information on an online software program. She will get program updated to include sterility assurance documentation.
- 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
- (1) There was a small bottle off oil that was not properly labeled in the hairstyling area. The label had come off/worn off. The operator used a sharpie to label the bottle at the time of inspection. * Ensure all containers are properly labeled.
