    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED APPROXIMATELY OVER 100 MICE DROPPINGS IN THE FOLLOWING AREAS. 20-30 IN CORNER AT FRONT CASHIER/COUNTER. THROUGHOUT BASEMENT STORAGE ALONG ALL BASEBOARDS AND ON SHELVING. INSTRUCTED TO HAVE PEST CONTROL CO SERVICE ESTABLISHMENT FOR MICE, REMOVE ALL MICE DROPPINGS AND SANITIZE ALL EFFECTED AREAS.
    • 19. OUTSIDE GARBAGE WASTE GREASE AND STORAGE AREA; CLEAN, RODENT PROOF, ALL CONTAINERS COVERED
      • The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. LARGE PILE OF EMPTY FOOD CONTAINER BOXES EXCEEDING 10 FEET HIGH PILED ON REAR OUTDOOR PATIO. INSTRUCTED TO REMOVE ALL BOXES, DEBRIS AND GARBAGE FROM PATIO AND MAINTAIN CLEAN AT ALL TIMES.
    • 30. FOOD IN ORIGINAL CONTAINER, PROPERLY LABELED: CUSTOMER ADVISORY POSTED AS NEEDED
      • All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED FOODS IN WALK-IN COOLER MUST BE LABELED AND DATED.
    • 32. FOOD AND NON-FOOD CONTACT SURFACES PROPERLY DESIGNED, CONSTRUCTED AND MAINTAINED
      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PLASTIC FLOUR BIN BROKEN AND REPAIRED WITH DUCT TAPE. MUST REPLACE. RACK SHELVING THROUGHOUT WALK-IN COOLER RUSTY AND WITH OLD FOOD ACCUMULATION. MUST CLEAN AND REMOVE RUST. RUBBER DOOR GASKET ON TALL REACH-IN COOLER TORN, IN POOR REPAIR. MUST REPLACE.
    • 33. FOOD AND NON-FOOD CONTACT EQUIPMENT UTENSILS CLEAN, FREE OF ABRASIVE DETERGENTS
      • All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. ALL INTERIOR AND EXTERIOR OF COOLERS AND FREEZERS MUST CLEAN. FOOD SLICER DIRTY WITH OLD ENCRUSTED FOOD DEBRIS. MUST CLEAN AFTER EACH USE.
    • 34. FLOORS: CONSTRUCTED PER CODE, CLEANED, GOOD REPAIR, COVING INSTALLED, DUST-LESS CLEANING METHODS USED
      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS UNDER AND BEHIND COOKS LINE EQUIPMENT WITH FOOD AND GREASE DEBRIS. MUST CLEAN AND MAINTAIN.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. SMALL THREE COMPARTMENT SINK NOT MAINTAINED. ALL THREE COMPARTMENTS MUST PROPERLY DRAIN. INSTRUCTED TO REPAIR AND USE LARGER THREE COMPARTMENT SINK TO WASH RINSE AND SANITIZE.
    • 41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
      • All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL UNNECESSARY ITEMS STORED THROUGHOUT BASEMENT MUST BE REMOVED TO PREVENT RODENT HARBORING AND ALL OTHERS MUST BE STORED ORGANIZED, ELEVATED FROM FLOOR 6" OR ABOVE AND 6" AWAY FROM WALLS.
    • 43. FOOD (ICE) DISPENSING UTENSILS, WASH CLOTHS PROPERLY STORED
      • Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. WIPING CLOTHS MUST BE STORED IN A CLEAN CONTAINER WITH SANITIZING SOLUTION. NOT ON NOZZLE OF EXPOSED HAND SINK.
