- Ants noted in back area.
- Minor activity for both pests.
- **Significant number of dead flies on floor, under triple sinks, in compressor area of freezer units, and in the internal cavity of the units. With the air movement around the various spaces in the back area there is a reasonable chance dead flies can get into the frozen food in the freezers.
- Corrective Action(s): The sink room needs a good cleaning of the dead flies as discussed.
- Continue to block access around grease trap on floor to limit drain fly breeding.
- Contact pest control to continue with ant and drain fly control programs.

- Observation: A) Significant ant activity noted throughout premises including:
- - Slushy area
- - dish sink room (floor walls and on boxes)
- - Back door area
- B) Significant wood mite (or similar) activity noted behind slushy machine area. Noted on walls and on floor.
- Corrective Action(s): A) Clean up and control sugar/food ants are attracted to in all noted areas. Make sure contaminated boxes and/or structures/surfaces cleaned or removed from premises right away. Provide a written plan of action to reduce ant population and keep under control in future. Have plan ready for follow up inspection.
- B) Control mites by keeping area dry. Remove mould andor decaying cellulose/wood from area. COntact pest control for advice and recommendations related to removal of these bugs. Discuss at follow up inspection. Develop action plan.

- A) Front service area of premises observed with accumulations of food debris in corners and along edges of floor counters and walls throughout. Mats observed dirty. Ceiling vent observed with build up of dust and cobwebs.
- B) Floor of dishsink room around beverage syrups observed with syrup accumulations. Boxes on floor also observed with syrup accumulations. Floor under freezers observed dirty. Area under and around dish sinks dirty. General floor in hard to reach areas dirty.
- C) Back chemical storage area and throughway for incoming foods dirty.
- D) Slushy area under wood supports on floor dirty. Adjoining spaces also very dirty.

- Observation: Some cleaning has taken place in identified areas noted in previous inspection, but adequate. Food debris remains under coolers, around shelving units and behind iced beverage stations as discussed.
- Corrective Action(s): Cleaning must be more complete in all areas noted in previous inspection as discussed.

- Observation: Frozen beverage station observed with significant build up of beverage syrups on floor in area behind machines. Syrups accumulating in space left by missing or broken tiles, under wood support and around edges of wall. The floor is not easy to clean due to poor construction design.
- Corrective Action(s): The back area of this station must be re-designed to allow cleaning staff to properly clean and maintain as discussed.
- 1) Replace missing tile and ensure the floor is easily cleanable.
- 2) Remove/replace wood support such that spilled liquid from the machines can be properly contained and cleaned on daily basis.
- Complete remediation/repair of this station should be completed in 60 days. Submit plan of action by email to this office within 21 days.

- Observation: Significant ant infestation noted in the back area of premises, around the mop sink. Ants coming out of gap at floor wall interface. Food debris and possible beverage spill in and behind the shelf next to mop sink.
- Corrective Action(s): General clean up of premises, including built up food and sugar spills will impact ant activity. Clean up. Move shelves as discussed to perform proper clean up.
- Block access points used by ants as discussed. Start at the mop sink area and expand as needed.
- Contact pest control contractor to actively control ant population.

- Observation: Drain fly infestation remains and flies observed in and around grease traps. Most notablely coming out of floor level grease trap at the gap in the lid. Also observed coming in and out of floor drain at grease trap, although likely just due to proximity to noted breeding environment. Flies also likely accessing upper grease trap, but difficult to confirm at this time.
- Corrective Action(s): The original floor level grease trap lid must be sealed to prevent drain flies from gaining access to this breeding environment.
- A) If this trap is not in use seal entire lid edge with silicone or similar. The upper grease trap will need to be moved to complete sealing process.
- B) If trap is still in use and requires periodic access for cleaning, install a rubber gasket around the entire edge of grease trap to prevent fly access.
- Please complete work in sixty days and send pictures to this office as confirmation work is complete.

Main concentration of flies around 3 compartment sink and grease trap. Blreeding cycle clearly not broken and daily controls measures for the drain and fruit flies inadequate.
- Corrective Action(s): A) Improve cleaning procedures for this room. Any ponding or stagnant water or beverage residues will allow breeding to continue.
- B) Continue to have Pest Control identify potential drain/fruit fly breeding habitat, and work to develop a plan to eliminate drain fly problem.
- **Send report from Pest Control detailing all potential breeding habitat in sinks/storage room and corrective/maintenace actions required to eliminate fly problem.

- Also noted the floor in the back mop sink area of the store accumulating syrpus and food debris around stacked pop and floor drain area.
- Corrective Action(s): Improvements must be made to the quality and detail of the daily cleaning. Any spills and accumulations of food debris should be cleaned up immediately with a general cleaning of facility at the end of the day. Cleaning schedules and processes must be clarified/adjusted to ensure the store is cleaner.
- **Pests issues can only be addressed when food sources are eliminated without delay.

- Observation (CORRECTED DURING INSPECTION): Sandwich cooler measured 11 C on one side of unit and 9.5 C to 10.5 C on the other side. Multiple levels checked. Unit briefly measured at 3 to 4 C. Issue remains. Last check measured 13 C. Staff immediately emptied unit of sandwiches and all potentially hazardous foods. Unit continues to be a risk for microbial growth.
- Corrective Action(s): Have serviced or replaced such that unit/food can maintain 4 C or less at all times. Do not stock with food until this office confirms unit can work as intended. Follow up in 1 week.

Integrated pest control practices must be adopted to eliminate this problem. Discuss grease trap (rim of grease trap) with Pest Control. Any control plan should include the grease trap/lid.

- Observation: Sandwich cooler measured 11 C to 14 C in multiple locations and with IR gun and proble thermometer. Reviewed temp logs from today at 7 AM and indicated 3.5 C. Checked multiple times during inspection. Discussion with operator that unit due for replacement.
- Corrective Action(s): Remove all potentially hazardous foods from this unit until it can consistantly maintain food at 4 C or less (at all times). Re-inspection required prior to use.

Action plan must include all areas of premises including floor drains, mop sink area, dish sink area/grease trap and iced beverage stations. Action plan to be submitted to this office within 10 business days. Corrective action to be completed within 30 days. Supply pictures of completed work to this office.
