- Observation: 1) On going mouse presence in premise. Pest control company currently coming in weekly. Daily cleaning of premise done to reduce access to food. Significant number of droppings observed behind drawers under displays and under oven. Observed a hole through metal flashing under front display nearest the bread slicer.
- 2) Numerous small flies observed in back area, mostly concentrated in drain between staff washroom door and proofer. This drain had a significant biofilm build-up.
- Corrective Action(s): 1) Patch up hole and do a deep clean under proofer. (due by May 8, 2024) Continue with weekly pest control and email their last report along with contact information to undersigned EHO.
- 2) Have back drain clean cleaned and treated for small flies at next pest control visit.

- Observation (CORRECTED DURING INSPECTION): Quats sanitizer dispenser line was empty and not dispensing any detectable level of sanitizer. Concentrate level in bottle was too low.
- Corrective Action(s): Bottle switched out during inspection. Monitor this daily to ensure a 200ppm concentration of quats at all times.

- Observation: Observed mouse droppings in drawer along where extra bread labels/signs are stored in the front. These signs are placed along side products on display and could reasonably come into contact with the food.

Staff state that they clean from corner to corner of the premise floor including under storage and equipment every 2-3 days. Pest control company in once a week according to staff however they did not have access to the reports.

- Observation: Observed another mouse hole in dry wall in staff washroom. This area was previously required to have a durable, pest resistant material installed along walls to prevent mice from entering in this area but this has not been done to date. Alternatively pest proof weather stripping under the staff washroom may be installed.
- Corrective Action(s): Install a pest proof material lining walls in bathroom or install pest proof weather stripping under staff bathroom door to prevent the movement of pests through premise. Correction order issued.

- Observation: Significant number of mouse droppings found throughout premise including behind bottom drawers in front display cases, under flour storage, bathroom, behind ovens, etc.
- Corrective Action(s): Thoroughly clean these areas again and continue to do so daily. Recommend supplementing pest control measures from pest control company with snap traps, though be mindful of placement as not to pose a hazard to staff. Check snap traps daily, discarding any captured mice.

- Observation: Two live mice observed during inspection, several mouse droppings found throughout premise but no food or food contact surfaces found impacted. Spoke to owner on site, pest control coming in weekly.
- Corrective Action(s): 1) Contact pest control company and have them do a detailed inspection of all potential entry points and have them provide recommendations for how to address the entry points found. Due: 1 week
