      • -door handle for the back of house sliding door cooler found to be repaired with duct tape
      • -walk-in cooler's floor is severely rusted and pitted
      • new: hose for back of house kitchen is broken and pitting
      • Corrective Action(s): Repair the above noted issues. 1 month extension for repairs granted, as requested by operator.
      • Provide photos via email to area inspector once complete.

      • Corrective Action(s): Repair the above noted issues. Duct tape is not a smooth, non-absorbent, and easy to clean surface and not permitted for use to repair equipment.

      • b) Install the UV light in the back that is unplugged. Consider implementing other measures to control the flies as well, such as installing UV lights in the front area.
      • c) Ensure that fly strips are kept outside of food prep areas. Discussed with the cook on site.
