- Observation:
- Observed minimal rodent dropping around the drain pipe for the ice making machine. The constantly dripping water is attracting rodents.
- Corrective Action(s):
- - Clean up the droppings, and wash & sanitize all surfaces that came into contact with the droppings.
- - Install wire mesh (1/4" or smaller) to cover the drain pipe to prevent access to the dripping water, and to prevent rodents from accessing the premises up through the floor drain.

- Observation:
- Contantly dripping taps at two prep sinks in the kitchen. The dripping water creates a water source and will attract pests.

- Repair the taps so they no longer constantly drip, and water can be fully turned on & off.

- Observation:
- Found rodents droppings on the floor around the small mixer in the kitchen; in the mop sink; and around the floor drain beside the upright cooler in the mop sink room. The rodents are being attracted to these areas as there is a constant water source (dripping water from condensor and backflow preventor.
- Corrective Action(s):
- Immediately clean up the rodent droppings, clean and sanitize all surfaces that came into contact with the droppings.
- Notify the pest company of these active areas.
- Increase the frequency the pest company is servicing the premises, as rodent activity was found.
- All reports/inspections provided by the pest company MUST be stored on-site, with the premises' Manager of Food Services. Such that, when the district EHO request to review the reports, they can be immediately produced.

- Mould growth found on the interior surfaces of the ice machine storage bin. Mould can contaminate the ice and could cause illness if consumed.
- Corrective Action(s):
- Wash and sanitize the interior surfaces of the machine to remove the mould. Add the cleaning and service of the ice machine to the cleaning schedule, so it is done regularily.

- Found chipped, cracked and/or damaged plastic food inserts and lids. These items are not cleanable and pieces of plastic could break off and contaminate food.

- Immediately discard any chipped, cracked, and/or damaged plastic food inserts and lids. Purchase new items to replace the damaged ones. Develop a process which staff will inspect food inserts and lids on a regular basis for damage. At time of the inspection, staff discarded the damaged food inserts and lids.

- Observation: Hand washing station at the dishwashing area has been decommissioned (tap handles, soap and paper towel dispensers all removed). Staff must be able to wash their hands between racking the dirty dishes and unloading the clean dishes.

- Observation: High temperature dishwasher in Dogwood East not working during inspection (display screen stated it was in the rinse cycle however it was not running) and could not be reset/repaired by staff on site during inspection.

- Observation (CORRECTED DURING INSPECTION):
- Food in the fridge, A1 kitchen, was 12C while the fridge temperature was 14C. Bacteria will grow in the food as it is above 4C.
- Corrective Action(s):
- Immediately decrease the temperature of the fridge. Check the temperature of the FOOD after 1 hour, if the food is above 4C then THROW AWAY all perishable foods (dairy, proteins, cut fruit/veggies, etc.). During the inspection, the temperature of the fridge was lowered.

- Observation (CORRECTED DURING INSPECTION):
- The pan/pot dishwasher had an empty soap dispenser. The pans/pots are not being washed properly, this will leave food, grease and bacteria on it.
- Corrective Action(s):
- Immediately put more soap in the dispenser, and ensure all staff recognize what the warning buzzer sound is. During the inspection, staff placed more soap in the dispenser.

- Observation:
- 1) A1 kitchen fridge temperature was 14C. Fridges must be kept at 4C or lower to slow bacterial growth.
- 2) A4 kitchen door handle of fridge is missing. It is difficult to open the door and this may damage other parts of the door or seal.
- 3) Found cracked or chipped plastic food inserts and lids and ice scoop. Bacteria can grow in the cracks, and/or pieces of plastic can fall into food or ice.
- Corrective Action(s):
- 1) Immediately lower the temperature of the fridge so it can maintain 4C or colder at all times. During the inspection, the temperature was lowered and will be checked in 1 hour. If the temperature has not decreased below 4C, immediately STOP using the fridge and have it repaired/replaced.

- A1 kitchen fridge had no thermometer in it. Staff are unable to monitor or record the temperature of the fridge daily, and therefore do not know if there is a problem with the fridge.

- Observation:
- Yearly health permits fees not paid. The facility is operating without a valid decal. Hand delivered the outstanding permit fees letter to the kitchen manager.

- Immediately pay the fees of $250 within 10 working days. Failure to do so will result in further enforcement action such as issuance of fines or closure of the facility.

- Observed large number of black flies in the dry storage room and a few, fresh rat droppings in the room with the special "dietary cooler". Fly traps, blue light, snap traps, and catch-all traps observed throughout the facility. Facility has monthly contract with Orkin Pest Control, reviewed the reports.

- Increase the visits from the pest control technician so traps are checked more frequently, and any caught pests are removed immediately to prevent flies. Rodent proofing of building should be done to prevent entry of pests.

- Observation: Display cooler in the cafeteria measured approximately 8C. Foods are not stored here overnight, mainly used when the cafeteria is open.

Until then, potentially hazardous foods may only be stored in this cooler for up to 4 hours before being discarded. Do not return foods to coolers after the 4 hour period. Obtain an accurate thermometer for this cooler. Send a photocopy of the repair invoice to your area inspector.
