      • Observation:
      • 1) Leaking pipe under the 2-compartment, dishwashing rinse sink. Water is pooling in the cupboard below and encouraging flies to breed.
      • 2) Water is pooling on the shelf the new dishwasher is sitting on. The water from the 2-compartment rinse sink and sprayer (beside the dishwasher) is dripping between the dishwasher and the counter top. The pooling water encourages flies to breed.
      • Corrective Action(s):
      • 1) Repair or replace the pipe so it no longer leaks.

      • 2) Fill, block, or caulk/seal the opening between the counter top and dishwasher to prevent water from dripping down and pooling on the shelf.

      • Observation:
      • The wall beside the upright freezer has chips and peeling paint exposing the drywall. The wall is longer sealed and cleanable. This is a REPEATED violation that STILL has NOT been CORRECTED.
      • Corrective Action(s):
      • Repair the wall so it is durable, smooth, level, sealed and cleanable. A follow-up inspection will be completed after the correction date. Failure to repair the wall will result in the ISSUANCE OF A HEALTH ORDER.

      • Observation:
      • Black tape found between counter top and left-side of dishwasher. The tape is not durable, cleanable, sealed or food grade. This will allow accumulation of water & food and attract flies and mice.
      • Corrective Action(s):
      • Remove the black tape and use a material that is safe in kitchens, durable, smooth, sealed and cleanable.

      • 1) The high temperature is constantly leaking, a bus pan is catching the water. Pooling water will attract and provide a water source for flies and mice.
      • 2) Metal piece on the interior door, of the dishwasher, is not secure. This is a safety hazard and could harm staff..
      • Corrective Action(s):
      • 1) Repair/replace the diswasher so it no longer leaks.

      • 2) Repair the metal piece so it is securely fixed to the interior dooor, and operates as per the manufacturer's specifications.

      • Observation:
      • The double doors, leading to the smoking area, do not self-close tightly as the doors do not line-up correctly. This causes the doors to be kept open, which will allow mice to enter. There also is a gap between the double doors that will allow mice and cold air to enter.
      • Corrective Action(s):
      • Immediately have the doors repaired so they fully self-close and are not kept open. Immediately install durable weather stripping between the double doors to create a seal that will prevent the entry of mice and cold air.
