Inspection summary
Crest View Care Village has been inspected 8 times by Interior Health Authority since August 10, 2016. The most recent inspection, on May 12, 2026, recorded 1 infraction.
1 of 8 inspections on record found no infractions at all. In the past 12 months there was 1 inspection with 1 infraction recorded. All recorded findings relate to upkeep, equipment or paperwork rather than direct health risks.
Recent inspections have recorded fewer infractions than earlier ones, so the record is improving.
What these findings mean
Health authorities word their findings differently, so we group them by what they mean for the people in care. Higher-risk findings are the ones that could directly make someone sick, and they matter more here because young children, older adults and people who are unwell are more vulnerable to infection.
Permits, records and postingLower risk
Operating permits, required signage, written procedures and logs. These findings are administrative: the documentation was missing or out of date at the time of inspection.
Recorded 9 times across 6 separate inspections, most recently on May 12, 2026. For example:
3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
Equipment and utensilsLower risk
Equipment has to be made of cleanable materials, kept in good repair and stored properly. These findings are mostly about upkeep, like a cracked surface or worn part. They are generally lower risk but show how well the place is maintained.
Recorded 3 times across 3 separate inspections, most recently on January 24, 2023. For example:
1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
Other findingsLower risk
Findings that do not fit the categories above, such as follow-ups that confirm an earlier violation was corrected, or requirements specific to this type of facility.
Recorded 5 times across 4 separate inspections, most recently on January 30, 2024. For example:
3 - Is documentation concerning restraints adequate?

- Routine Inspection
1 infraction
- R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Observation(s): RCR 81(2)(b) - A review of a care plan identifies that it does not include a plan for a person in care who is currently accessing the community independently. The absence of documented care planning for independent community access increases the risk that a person in care’s abilities, safety needs, and required supports are not adequately identified or addressed. Without clear planning and oversight, there is potential for unmet needs, increased risk of harm, and care delivery that does not align with assessed capabilities or licensing requirements. Submit by May 27, 2026, a written plan outlining how the contravention related to ensure care plans take into account the unique abilities, physical, social and emotional needs and preferences of persons in care has been addressed. The plan must also describe the system and processes that will be implemented to support the ongoing monitoring of Section 81(2)(b) of the Residential Care Regulation, thereby ensuring sustained compliance moving forward.
- R10.3A - Develop a care plan with the participation of the person in care to the extent reasonable practical or the parent or representative and takes into account the unique abilities, physical, social and emotional needs, cultural and spiritual preferences of the person in care; 81( 2 )(a)(i)(ii)(b) (Show More)
- R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Routine Inspection
1 infraction
- R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Observation(s): R10.3J - During a review of wound care plans, it was noted that two of the four care plans did not include clearly defined instructions for frequency of completion. The absence of clear written instructions regarding the frequency of wound care increases the risk of inconsistent treatment and potential delays in care. It also presents a risk of miscommunication among staff and a lack of clarity regarding care responsibilities. Submit by May 12, 2025, a written plan outlining how the contravention related to wound care plans has been addressed. The plan must also detail the systems and processes that will be implemented to support ongoing monitoring of care plans as required by Section 81(4)(a) of the Residential Care Regulation, thereby ensuring sustained compliance moving forward. R10.3K - During the review of care plans, it was identified that one care plan had not been updated to reflect the person in care's current mobility status. Failure to update a care plan can result in care that does not reflect the person's current needs or condition. This increases the risk of staff not having accurate guidance to provide safe and effective care. Submit by May 12, 2025, a written plan outlining how the contravention related to updating care plans has been addressed. The plan must also detail the systems and processes that will be implemented to support the review, and if necessary, modification of care plans as required by Section 81(4)(b)(i)(ii) of the Residential Care Regulation, thereby ensuring sustained compliance moving forward.
- R10.3J - Each care plan must be monitored on a regular basis to ensure proper implementation; 81( 4 )(a)
- R10.3K - Review and, if necessary, modify each care plan if there is a substantial change in the circumstances of the person in care, or at least once a year, to ensure it continues to meet the needs, preferences, and is compatible with the abilities of the person in care; 81( 4 )(b)(i)(ii)

- R10.2 - Do care and supervision practices ensure health, safety and dignity of persons in care?
- Observation(s): The system to ensure the health and safety of persons in care is monitored regularly to determine if their needs continue to be met is ineffective. It was identified during the inspection that the 'smoking assessment' is not being utilized to monitor the ongoing health and safety, and determine if the needs of persons in care who smoke are being met.

- Observation(s): Upon review of the facilities record of complaints, it was noted that the licensee is not currently retaining a record of responses to complaints and concerns.
- R4.6F - Retain a record of complaints made and concerns expressed under section 60 (dispute resolution) and the responses to them; 89 ( 1 )
- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?

- R10.2 - Do care and supervision practices ensure health, safety and dignity of persons in care?
- Observation(s): Licensee is noted to complete consistent monthly room audits. Upon review of the audits it was noted that a repeat hazardous item is being removed on a regular basis from one room. Licensee does not currently have a system in place to mitigate this risk. During a random chart audit, it was noted that persons in care's charts did not have persons authorized in writing to remove or release a person in care from the facility.
- R10.2A - Monitor the health and safety of each person in care regularly to determine if their needs continue to be met; 50 ( 1 )
- R10.2Q - Only a parent or representative or a person authorized in writing by that person can release or remove a person in care from the facility; 58( 1 )(a)(b)
- R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Observation(s): During a random chart audit it was noted that the system for ensuring all care plans were updated appeared ineffective. This was noted by a detailed care intervention missing a date, as well as the person in cares name not being present on the form. This was also evidenced by an outdated smoking care plan.
