     • Observation: Observed one of the tube light fixture/base was missing the cover for the light. As these lights were assumed to be non shatterproof lights this can pose a risk for physical hazard and contamination of food items and food contact surfaces if the light were to shatter.
     • Corrective Action(s): The operator and staff were instructed to repair or replace this unit so that it is shatterproof and/or covered to prevent physical contamination risk. Operator/staff stated they will contact the maintenance team regarding this.

     • Observation: 1.Tile under dishwashing sink had tape around perimeter, baseboard underneath dishwashing sink peeling off and has tape trying to hold it up, paint chipping off of window sill above dishwashing sinks.
     • 2. Left chef cooler door seal falling off and being held on with two pieces of tape. Staff are aware of issue and ensure door is closed properly. Temperature log of cooler shows adequate temperature levels.
     • Corrective Action(s): 1.Remove tape from tile and clean area, clean around baseboard and properly attached baseboard (no tape), repaint window sill. Correct all within one month.
     • 2. Try to adhere seal with glue. Replace as needed.
