      • Corrective Action(s): Have pest control company service the premises and apply proper pest control measure immediately. Increase sanitation as per comment on code 306. If the old drive through window is opened occasionally, please install a window screen to prevent pest entry.

      • Observation: Lots of food debris on food prep flooor (e.g. ice cream machine area, under deep fryer and walk-in cooler) and some pooling of water under food prep table of gravy station and walk-in cooler from condenser dripping water. Ice machine shows some mildew.
      • Corrective Action(s): Deep clean the facility so that food debris and standing water are removed. Clean and sanitize all surfaces. Repair the condenser if it continues to drip water. Poor sanitation is a major contributor to fly issue. Clean the ice machine.

      • Observation: Handsoap dispenser is loose so that pressure must be applied to the hold the dispenser in place for soap to come out.

      • Observation (CORRECTED DURING INSPECTION): An insert of mushroom sauce appear to have white bubbles and white/yellowish powder like substance on the inside of the insert. Inquired manager how mushroom sauce is prepared. The prepackaged mushroom sauce come frozen and gets thawed in walk-in cooler overnight then microwaved next day to be used and stored in ice on the line by deep fryer. The new pack of frozen mushroom did not appear to have any white substance in the mushroom sauce itself. Photos taken.
      • Corrective Action(s): Mushroom sauce was discarded. Ensure to use clean inserts to store food and discard when food appears to be contaminated or mouldy.

      • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation: 1) One bucket of Quats sanitizer was measured at 100ppm. 2) The ice bin appear to have mildew on the corner. When paper towel was used to wipe down the mildew like area, brown substance was removed. Repeat. Ice bin was informed by manager to be cleaned nightly. 3) The blizzard metal ring holders are informed to be rinsed off at dump sink in the front, then soaked in water and then sanitizer before next use. No drying step noted. Two buckets with metal rings soaked inside measured 100 and 200ppm Quats.
      • Corrective Action(s): 1) Please ensure that there is 200ppm Quats sanitizer for use at all times. 2) Discard ice. Ice bin needs to be cleaned and disinfected more carefully and mildew and build up is common around corners of the bin. 3) Please send section in sanitation plans regarding procedure to clean and sanitize metal ring holders for blizzard.

      • Observation: 1) One insert of condiment in the walk-in cooler was not covered. 2) Several boxes of frozen food stored on floor of walk-in freezer. 3) Raw frozen burger patties were stored in the chest freezer directly or on the wire rack.
      • Corrective Action(s): 1)Insert was properly covered at time of inspection. 2) Food must be stored 6 inches off the ground. 3) Use food grade containers or bag to store food so that it is protected from contamination.

      • Observation (CORRECTED DURING INSPECTION): Found two plastic caps for blizzard cups being stored with metal blizzard ring holders soaked in Quats sanitizer. One gold and one red spoon with water droplets were stored in blenders that appear to be just washed.
      • Corrective Action(s): Do not use single use items as they are one time use and must be discarded after use. All noted items discarded.
      • Violation Score: 3

      • Observation: Wall tiles under dishwasher area and around mop sink still loose or missing. Ceiling light upstairs outside of office area is out.
