- Multiple sanitizer solutions were tested and were found to be 50 ppm or less. Suspicion of PHI that these were made using the dispenser at the front which is only dispensing at a concentration of 50 ppm. All solutions were remade from the dispenser at the back at the request of the PHI.

If this cannot be corrected in a reliable fashion, discontinue usage of this dispenser and use the sanitizer dispenser located at the back.

Discussed with staff present and showed the areas of concern. explained that this could be an attractant for vermin as well as insanitary conditions.Requested that they send pictures once the work was completed.Mar 21, 2024Floors throughout the facility require cleaning with special attention to corners and underneath/behind equipment. Including inside the walk-in freezer.
