      • Debris observed on cooling fan covers in walk-in cooler.
      • Debris observed on can opener.
      • Corrective Action(s): Sanitizer dispenser to be serviced. Observe sanitizer concentration for above 200ppm as per test strip. Use alternate sanitizer 1oz bleach per gallon water.
      • Clean and disinfect cooling fan covers in walk-in cooler.
      • Date to be corrected by: April 30/21

      • Observation: Beverage syrup bags stored directly on floor in back storage area.
      • Corrective Action(s): Make sure such practice is stopped. Store beverage syrups off the floor at all times.

      • 304 - Premises not free of pests [s. 26(a)]
      • Observation: 1) Ants noticed below dish and pot sinks in kitchen. Noted heavier activity at bottom of cabinetry bordering single pot sink (picture) and under unused water-soaked particle board shelf leaning in this space. Removed this shelf. Also observed the tile grout on floor pocked with holes in this area and to the left of the grease trap.
      • 2) Drain flies noted in the dish sink area (minor), but a small concentration observed along the floor of the above noted area, including coming out of the pocks in the grout.
      • Corrective Action(s): Have Pest Control continue to take action against both ants and drain flies as discussed. Staff must also work to eliminate standing water on floor. Target grout lines in floor tile.
      • **Investigate the extent of the holes in groutwork in area and fix as necessary. Possible source of ant entry and drain fly breeding.
      • Contact this office with action plan.
