      • Pictures taken at this time. Traps in place. Facility does not have a professional pest control company contracted.
      • Corrective Action(s): Ensure to have ALL dropping removed. Manager had indicated droppings were not cleaned for a week. This is unacceptable.

      • Manager had advised piles or rice were mixed with poisonous chemicals and purposely placed at these locations.
      • Corrective Action(s): Ensure to have these removed immediately as discussed at this time.
