- The measured temperature at the handwashing stations accessible to children was 55C. A recommended maximum of 49C and an absolute maximum of 55C must be observed. Please have a licensed professional plumber provide solutions. Reducing the overall temperature of the water heater is not permissible, as the dishwasher is sanitizing based off high temperatures.

- The child proofing strap located on the cabinet below the handsink in the preschool bathroom was in disrepair. New adhesive was applied to the strap for immediate repair to prevent child access to cleaning items/chemicals.
- 06. Are proper protocols being followed in regards to client care?
- The safety belt on the diapering mat in the infant room was observed to have fallen off and was in disrepair. The old diapering mat was discarded and replaced with a new diapering mat with a suitable safety belt.

- A large piece of cardboard was taped to the change table in the infant room. The cardboard and tape was removed upon request. Ensure surfaces are smooth, durable, and impervious to moisture to facilitate ease of cleaning.
- 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
- An elevated climbing structure that measured 33 inches in height was observed in the toddler room. The climbing structure was removed immediately. Indoor playground surfacing requirements were sent to the operators.

- Several jackets and snow pants were observed to be stored next to each other with the items directly touching. The items were moved further away such that the items were not touching. Ensure personal belongings have adequate separation and do not directly touch while in storage. A large bag can be used to store items for separate storage.

- The flush handle for the left toilet in the toddler room was missing. Replace the missing flush handle and ensure the toilet is functioning properly.
