      • PREVIOUSLY - You are to have a written Pest Control program.Part of this program can be the use of the attached - Pest Control Checklist.Please complete the checklist on a monthly basis, while you are in operation and keep the completed list in a file or binder for future review by PHI.May 26, 2025 - staff were not able to find any completed Pest Control Checklists during the inspection.Please email copies of last year's and this year's completed Pest Control Checklists - to your PHI.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • PREVIOUSLY - The current food permit is not displayed.So please check your emails or contact our Food Permit Administrator Lisa at 1-833-476-4743, to obtain a replacement permit.May 26, 2025 - this was corrected last year, as the Food Permit on the wall expired in 2024. Please get your current Food Permit posted.Email photo of posted current Food Permit to your PHI.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • The arborite on the kitchen countertop, to the right of the sinks, the seam has split and is lifting.This will need to be repaired or replace the countertop with new material.Send photo to PHI of completed work.

      • Mouse droppings were observed on the shelf area under the kitchen sinks.Repair this water damaged bottom shelf, under the sinks. Fill in any gaps around plumbing pipes with steel wool. Place mouse traps or sticky boards to capture the mice, if still present.

      • There is a new, bare wood, shelf on the west wall of the kitchen.The bare wood needs to be sealed or painted to make cleaning easier and prevent slivers from getting into food handler's hands.Note - the main flat surfaces have a lacquered finish, so only the bare wood edges remain to be sealed/painted.
