    • 05. Are communicable diseases at this facility being properly managed?
      • 1. Facility has being using quats to clean and disinfect some surfaces, which is not effective against non-enveloped viruses such as norovirus and adenovirus. This increases the risk of infection transmission during a GI outbreak. Corrective action: A chlorine solution of 1000 ppm or a 0.5% accelerated hydrogen peroxide product must be used during an outbreak to ensure inactivation of pathogens. If using chlorine, test strips with a range up to 1000 ppm must be obtained to verify the strength of disinfectant solutions. 2. Cleaning cloths were not being used and replaced in a manner that prevents cross-contamination and illness transmission. Director indicated that they will use single-use, disposable paper towels going forward.
    • 06. Are proper protocols being followed in regards to client care?
      • Facility described doubling their gloves during diapering, which can create microtears in the gloves or soil them in the process of removing the top pair. This increases the likelihood of cross-contamination. Corrective action: Director was advised to discontinue double gloving immediately. At this time, the previous procedure involving 3 sets of gloves may continue so long as an alcohol-based hand rub is applied between glove changes. In addition, it is highly recommended that the sets of gloves required for the diapering procedure are removed from the bulk supply prior to starting to reduce cross-contamination risks.

    • 06. Are proper protocols being followed in regards to client care?
      • Standing diapering is being done at the facility, which differs from approved diapering procedures. Staff reportedly change sets of gloves 3 times throughout the procedure without doing hand hygiene in between. This practice interferes adequate infection prevention and control.Corrective action: It is recommended that staff wear only one set of gloves during diapering of each client, and use a tongue depressor or a similar implement to dispense creams or ointments. A baby wipe can be used to remove heavy soils should any come into contact with the gloves. Handwashing must be done after glove removal.
