- Corrective Action(s): Fix and properly stocked both primary and secondary hand sink on cookline as discussed.

- Observation: Mouse activity noted in kitchen. Droppings noted on carboard barrier on shelf below prep table. Activity noted to the right of the washer/dryer - insulation noted likely from water pipe insulation, and mice may be using this for movement to and from the ceiling. Minor activity noted (maybe old) in bar area, although this area is improved. Minor activity noted on floor of meat slicer station.
- Corrective Action(s): Continue to keep the kitchen spaces clean and free of mouse droppings. Block access where insulation and other signs indicate mouse activity as discussed. Continue to work with pest control.

- Observation: - Walk-in beer cooler floor is getting too dirty. Food debris and broken glass noted. Most build up around legs of shelves and in corners or hard to reach areas, but as this is a food cooler as well as beer cooler needs to be cleaner.
- - Walk-in freezer observed with a fair bit of food debris on floor.
- - Walk-in cooler is gnerally pretty clean, but pickle jar spilled on floor (broken?). Manager cleaned up during inspection.
- - Dish pit observed with debris build up in corners under service table.
- - floor around deep fryer observed with food and grease build up (minor).
- Corrective Action(s): Clean up above noted areas as discussed. Increase cleaning frequency in beer walk-in as it also serves as food cooler.

- Observation: Mouse activity much reduced, or less notable. Washer dryer space has minimal activity. Kitchen area below hot holding units and along the cookline improved.
- *Bar area including the counters, some cupboards and at the credit card station has notable activity.
- Corrective Action(s): Continue to work with pest control to improve the bar area mouse activity.

- Observation: Observed mouse droppings in employee washroom, at vacuum packing station, and to lesser degree in other areas of premises. Met and discussed issue with pest control operator. He stated much improvement. Visits weekly. Provides recommendations to restaurant. He is struggling with inability to use proper poisons on regular basis.
- Make sure basement door is easily closable and bottom gap is minimized to preclude mouse entry from this location.
- Corrective Action(s): Continue with pest control initiatives including thorough cleaning and sanitation, and access control.

- Observation: The following areas were noted dirty and require attention:
- - Walk-in cooler floor under shelves observed with food enriched stagnant water. Difficult to see as this space has marginal lighting.
- - Vacuum packaging and slicing station. Shelves and floor observed with lots of mouse droppings.
- - Under and around washing machine. Stagnant water observed and mixed with lint. Discussed with staff that leak is from drain pipe overflow.
- - Bar walk-in cooler. Ledge at bar beverage access observed with accumulation of brown substance.
- Corrective Action(s): Improve cleaning quality or frequency in above noted areas.
- Increase lighting in food walk-in cooler.
- Improve drainage from washing machine. If this can't be fixed washing machine will need to be removed. Review at next scheduled inspection.

- Observation: a) Washer/dryer station is a mess. Floor under and behind machine covered with a mixture of lint and stagnant water and has been left to accumulate. Biomat has formed which can be an attractant to pests. Discussed with manager and it's an issue of overflow during the drain cycle.
- b) Area behind the tiolet in the employee bathroom also observed with accumulations of stagnant water, which has formed a biomat.
- c) Bar floor under sinks dirty and bottle caps litter the space, with minor amount of mouse droppings. A few of the shelves also noted with minor amount of mouse droppings.
- d) Garbage enclosure and area excessive. Needs a cleaning and to better manage the bird pest population the lids must be kept closed.
- Corrective Action(s): Identified areas need to be cleaned up more often and better. Any shelves used for food or food containers should be cleaned and sanitized immediately.

- Observation (CORRECTED DURING INSPECTION): Quats sanitizer in spray bottles measured in excess of 500 ppm. None labelled. Staff likely using the sink sanitizer pump unit for spray bottles. This method is not appropriate for small volume containers.
- Corrective Action(s): Bottles were dumped and replaced with proper concentration of sanitizer (200 ppm) for food surfaces. Get a better dosing system for measuring sanitizer into spray bottles. Typical ratio is 1/2 tsp quats concentrate to 500 ml water, but follow manufacturers instructions.

- Observation: Mouse activity noted in many areas of bar and at gap of walk-in in back of kitchen as discussed.. also noted on secondary counters/shelves, on floor under plumbing fixtures etc. Mouse traps observed, a couple of traps observed with dead mice. Food and debris noted in similar areas.
- Corrective Action(s): Make sure Pest COntrol technician is setting and removing traps when needed, and that the traps are set properly.
- It is important to clean the floor in this area to support pest control practices.

- Observation: A) Moderate to significant volume of mouse droppings observed at and around bar, including on counters.
- B) Moderate vollume of mouse droppings observed on floor at and around washer dryer, and under slicer prep station on opposite side of this area.
- C) minor volumes of mouse droppings noted on pot shelves below work counters covered with cardboard and tinfoil. Also noted in corners and hard to reach areas of kitchen.
- D) Significant drain and fruit fly populations noted at bar glasswasher and main dishwasher stations. Attractions include dirty stinky rags left to accumulate on dolly, stagnant water or beverage residues on container lids, stagnant water under and around dishwasher and floor in area.
- Corrective Action(s): A) The counters and adjoining work surfaces or shelves must be cleaned daily or more. No mouse dropping should be evident during the day, or if noticed signifies active infestation and further investigations. Get staff to use flashlight or obtain brighter lighting for clean up at the end of day in the bar area.
- B) Move washer/dryer to evaluate this space for mouse activity, access, and/or food availability. Continue to block holes torestrict movement as discussed. Improve cleaning quality and/or frequency at slicer station.
- C) Remove foil and cardboard as shelving interface anywhere in premises and incorporate improved daily cleaning as discussed.
- D) Bait populations as discussed. Remove cloths/cover cloths. dry up all puddles of water below glasswasher and dishwasher as discussed.

- Observation: Basement - Walk-in cooler floor is dirty and neglected. Floor is in marginal condition, preventing proper cleaning. Floor outside walk-in, stair landing area, and stair slip grates dirty and need cleaning. Floor mostly unfinished.
- Corrective Action(s): Plan for repainting and cleaning of surfaces in similar manner to beverage cooler upstairs. Clean stair grates and stairs right away.

- Observation: Mouse droppings continue to be observed in the premises. Reviewed Pest Control Operators (PCO) report and recommendations for continued presence (2x/month) until population under control. Overall clean up of facility has occurred and active rodent activity and movements can be better traced.
- Corrective Action(s): Continue to prioritize mouse population control, entry and movement control by blocking key access points in bar area, kitchen at washer dryer and dough mixing station, at walk-in freezer and basement as discussed. Operator to also target exit doors to reduce easy access for mice.
- Continue to improve daily cleaning to reduce food sources as part of typical Integrated Pest Management Techniques as per PCO advice.
- Ensure PCO is present in facility at necessary frequency as a key tool to keep rodent population substantially in check and away from food prep environments.

- Observation: Significant cleaning has occurred in premises. More is needed in the following areas:
- A- Dishpit as discussed. Under and around stainless steel surround and under-counter extending to walls and floor. Biomat remains, although to a much smaller degree.
- B- Deep fryer station much improved, but accumulations of food debris and grease noted on floor between fryers and on the sides of the fryers themselves (may require scraping to remove such accumulations).
- C- Beverage cooler walk-in floor the same. Discussed with staff and operator. Plan to thoroughly clean floor and area, then refinish to allow proper and easier routine clean up.
- D- Ceiling tile including the HVAC exhaust air exchangers much cleaner.
- E- Gap to the left of the walk-in freezer door observed with food and non-food related debris. Discussed.
- Corrective Action(s): Continue to make improvements to the observations:
- A- Continue to explore means to improve cleaning to this area. Discuss alternate cleaning products with chemical supply company to facilitate easier and more complete end-of-day cleaning.
- **Seal all seams/joints at stainless steel surround at dishwash station to reduce water infiltration along wall to behind and below this station.
- B- General cleaning needed. Add procedure to revised sanitation plan.
- C- Send pictures of completed work.
- D- Ceiling tiles to be replaced. Send pictures of completed work.
- E- Send picture of completed work.
- Review above at next scheduled inspection.

- Observation: Rail and River lacks separate Health Operating permit. Discussed with management.
- Corrective Action(s): Obtain valid Health Operating permit by completing the application as discussed. Will be reviewed at next scheduled inspection for Pub and Grill.

- Observation: Mouse activity noted in several areas of premises, including the bistro dining area, the kitchen and bar area of premises. Did not evaluate the downstairs area of premises at time of inspection, but will be reviewed in future.
- Corrective Action(s): Premises must be completely cleaned up as noted in other sections of this report. Conce complete, have Pest Control company provide comprehensive report with recommendations to control/eliminate mouse activity. Ensure Integrated Pest Management techniques are implemented and maintained under the advice of the Pest Control Operator.

- Observation: Following areas of premises observed excessively dirty and not properly maintained including the build up of food debris, rags, greases, clothing etc:
- Bar - Internal cavity/shelves etc. of under counter cooling units; Shelves below bar; floor (very dirty);
- Kitchen - Floor everywhere! Under shelves; under grill; behind ice machine; around grease trap; below under counter coolers (less observed); at deep fry station; ceiling around main exhaust; especially around fire supression system; around dough mixer including pots and tray shelf (mouse droppings); station and washing machine area; Walk-in cooler shelves; beverage walk-in cooler floor; Staff washroom floor; cavity extending around walk-in freezer; Dishpit (mould, slimes and biomat development noted everywhere);
- The bar area of the Rail Bistro also observed with similar build up of food debris and general cleaning neglect on floor, under and inside equipment.
- Corrective Action(s): A thorough and complete deep cleaning and facility maintenance reset is required to identified areas above. Due to the extent and degree of neglect to facility contracting a third party to complete the cleaning portion of work required is recommended.
- The cleaning porttion of the overall work to this facility will be reviewed at follow up inspection date noted on report.
