      • No mop/utility sink on site. Instructed facility to install and maintain with hot and cold running water under city pressure. Observed a three compartment sink on site (no food prep or washing of dishes) with a faucet neck unable to reach all compartments. Instructed facility to install and maintain a faucet neck that is capable of reaching all three compartments.

      • Observed a quarter inch gap at the bottom edge of the front double doors where both doors meet. Instructed facility to repair so doors are tight fitting and to maintain. Additionally found 5 mice droppings near the rear exit door and 10 mice droppings near the three compartment sink. Instructed facility to remove and maintain. Recommend to contact a licensed pest control provider for service. Serious violation 7-38-020.
