      • Observation: 1) There are some loose/cracked/missing floor tiles and damaged baseboard under the back handsink that's attached to the 2-compartment sink. As per operator, this is due to water damage from the staff washroom flooding. Operator has already contacted mall management to have the issue addressed.
      • 2) There is no garbage can in the back. Operator has moved it to the front to prevent staff from throwing food debris in it since it attracts flies.
      • Corrective Action(s): 1) Replace/repair the floor tiles and baseboard.
      • 2) Provide a garbage bin in the back. You can purchase ones that have a lid that's operated using a foot pedal.

      • Observation (CORRECTED DURING INSPECTION): Pizza slices for the TGTG program are being stacked one on top of the other in the walk-in cooler. This will hinder the cooling process.
      • Corrective Action(s): Do not double stack pizzas one on top of the other. Different time stamped pizzas should be cooled separately. After 4 hours if they're at 4 degC, they can be covered or boxed and stored separately from the "new" pizzas coming into the cooler for cooling purposes.

      • Observation (CORRECTED DURING INSPECTION): Once again, when staff was asked to go over their dishwashing procedures, the sanitizing step was being skipped.
      • Corrective Action(s): This is a recurring issue and if it continues to be an issue, then a Violation Ticket will be issued. Educate all staff members on the proper dishwashing procedures and managers should ensure that staff are following these procedures.

      • Corrective Action(s): Purchase a measuring cup and determine how much bleach is required for the amount of water used during dishwashing to get 100ppm. Train all staff on this.

      • Corrective Action(s): 1) Start washing dishes as discussed. Follow your Sanitation plan.
      • 2) Purchase some sink plugs to enable dishwashing. This was brought up in a previous inspection as well.
      • 3) Ensure that all sanitizer spray bottles have 100-200ppm bleach solution in them.

      • Observation: Observed staff using the same pair of disposable gloves to carry out multiple tasks.
      • Staff were not wearing cloth masks properly and were constantly adjusting them.
      • Corrective Action(s): Disposable gloves shall be changed between tasks. Before putting on new gloves staff shall wash their hands with liquid soap and paper towels. All staff are issued cloth masks with company logo on them, however they do not appear to fit the faces of all staff persons properly. One staff person was observed wearing the cloth mask without it covering their nose, and they were constantly adjusting the mask. Proper fitting masks shall be provided to staff so that their noses and mouths are covered and do not require constant readjustment. Recommend operator provide cloth or single-use disposable masks that have a metal band that can be molded around the bridge of the nose for a more proper fit, and different sizes for different staff.

      • Observation (CORRECTED DURING INSPECTION): Mouse droppings found behind fountain pop dispenser and in the back corner of store by the lockers.
      • Corrective Action(s): Clean up mouse droppings and sanitize the affected areas. Mouse droppings were not found anywhere else. Premise is serviced by a licensed pest control company every 4-6 weeks (service is arranged by mall management). Operator cleaned up droppings at time of inspection.

      • Observation: Temperature inside walk-in cooler was 7.7 degrees Celsius.
      • Corrective Action(s): Cooler shall be capable of maintaining foods at 4 degrees Celsius or less. Air temperature and temperature of foods were found to be 7.7 degrees Celsius as per min/max probe thermometer. Temperature on the built-in temperature guage indicated 8.0 degrees Celsius. It appeared that staff had been going in and out of the walk-in cooler as they were preparing pizzas. Informed operator to monitor the temperature for the next two hours and if it does not return to 4 degrees Celsius or less, the walk-in cooler shall be serviced/repaired. Near the end of the inspection the temperature guage indicated 7.3 degrees Celsius. According to operator, corporate policy requires monitoring and recording of temperatures for cold holding units twice per day - once at opening and once at closing of store. Advised operator to also monitor and record temperatures of all coolers and freezers in the middle of the day to get a more representative understanding of how the temperatures may fluctuate, and to catch and address problems earlier.

      • Observation (CORRECTED DURING INSPECTION): Clean tongs stored directly above and in close proxiity to dishwashing area.
      • Corrective Action(s): The first compartment of the two-compartment sink is being used by operator for cleaning and the second compatment for rinsing and sanitizing. Items stored directly above the first compartment may become contaminated due to splashing and need to be moved away from this area. Operator moved the tongs and other items further away from the washing area.

      • Corrective Action(s): Clean up the mouse droppings and sanitize the affected areas. Premise is serviced regularly by a licensed pest control company. No signs of mice were observed anywhere else in the premise. Operator cleaned mouse droppings at time of inspection.

      • Corrective Action(s): 1. Bleach sanitizer must be 200ppm. Operator indicated the bleach sanitizer was prepared 2-3 weeks ago. Bleach sanitizer must be monitored to ensure adequate chlorine sanitizer concentration and changed regularly to ensure the concentration is always at 200ppm. Operator corrected at time of inspection.
      • 2. In-use wiping cloths shall be stored in sanitizer (e.g. the same bleach sanitizer found in the spray bottles) when not in use. Bacteria can grow in/on wiping cloths that are not stored in sanitizer. Alternatively, you may switch to using single-use/disposable paper towels instead of wiping cloths.

Adjust temperature setting to ensure cooked pizzas are held at the appropriate temperature. Corrected at time of inspection. Operator increased the temperature and during the inspection the temperature began to increase.
