      • Corrective Action(s): Due to a mouse infestation, ensure ALL food contact surfaces, equipment and utensils onsite are washed and sanitized. Rodent urine/feces can contaminate surfaces/equipment. Date to be corrected by: September 10, 2024.

      • - Dry storage (back area storage cabinet, back storage room)
      • - Equipment/utensil storage areas (all)
      • - Floors (along walls).
      • Corrective Action(s): Closure order issued. Ensure above noted areas are thoroughly cleaned to remove all rodent droppings and then sanitized.
      • Have premises serviced by a licenced pest control technician. Identify and seal potential entry entry points.

      • Observation: One hole was observed in the wall near the floor, located in the back area to the right of the electical panel. The hole was sealed with steel wool, but only partially/loosely (steel wool appears to have been placed there a long time ago).
      • Corrective Action(s): Ensure this hole is effectively sealed against pest entry. Monitor for new pest activity and reseal and locate any additional holes/gaps (if present) as needed. Date to be corrected by: February 28, 2024.

      • Observation (CORRECTED DURING INSPECTION): Four small containers of cooked rice (not acidified) were observed to be stored at room temperature. It was stated that rice was leftover from yesterday, cooled in the refrigerator overnight, then taken out of the refrigerator over two hours ago. Internal temperature of rice at 16 to 18 degrees C.
      • Corrective Action(s): Discarded. Ensure potentially hazardous food (including cooked rice that is not acidified) is not stored in the danger zone (4 to 60 degrees C) as pathogens can multiply rapidly at these temperatures, and may potentially produce toxins that may not be destroyed by reheating.
      • Store potentially hazardous food at either 4 degrees C or less, or at 60 degrees C or hotter in order to limit pathogen growth.

      • Observation (CORRECTED DURING INSPECTION): Hand washing sink was obstructed by a jug containing water. In addition, no soap was available.
      • Corrective Action(s): Liquid soap refilled and obstructing item removed. Ensure that at all times, all hand washing stations remain unobstructed and are supplied with hot and cold running water, liquid soap, and paper towels in order to facilitate proper hand washing.

      • Observation: Mouse droppings observed in some storage areas, particularly hard-to-reach areas (e.g. behind prep cooler). No food was observed to be damaged or contaminated at time of inspection.
      • Corrective Action(s): Ensure all affected areas are properly cleaned and sanitized today. Ensure equipment/supplies that are not being used for the business are either removed from the premises or stored in a manner that facilitates effective cleaning (for example, store in a covered bin) to minimize potential pest harbourage areas. Management informed that pest control has already been contacted and is scheduled for a visit on February 20, 2024. Ensure potential entry points are identified and sealed against pest entry.

      • Corrective Action(s): 1. Contents of spray bottle discarded. Sanitizer was remade and tested at 100 to 200 ppm chlorine bleach (in two spray bottles). Ensure sanitizer is maintained at concentrations required for safe and effective sanitizing of food contact surfaces (e.g. 100 to 200 ppm chlorine bleach). Spray bottles labeled.

      • Observation (CORRECTED DURING INSPECTION): Both vegetable slicers were found to have dried food debris from previous use.
      • Corrective Action(s): Ensure all equipment is cleaned and sanitized after each use. Equipment needs to be presoaked prior to washing to removed dried food debris. Items were properly washed and sanitized at the time of inspection.

      • Observation: Operator had a bar of soap in the back area and staff washroom did not have any single use paper towels.
      • Corrective Action(s): Bar soap is not to be used. Handwash station must be easily accessible and supplied with hot and cold running water, liquid hand soap, and single use paper towels. Bar soap may result in cross contamination. Bar soap was removed and paper towels were refilled at the time of inspection.

      • Observation (CORRECTED DURING INSPECTION): Sushi chef was observed using thumb to pick nose, followed by handling cut cucumbers without proper handwashing.
      • Corrective Action(s): This is direct contamination of foods. Hands must be washed before handling foods. Cucumbers were discarded at the time of inspection and sushi chef washed his hands prior to handling foods. All staff are to review proper hand hygiene practices. This is a repeat violation with regards to hand hygiene. Continued violation regarding hand hygiene will result in the issuance of a violation ticket.

      • Observation: Tin cat by the deep fryer had a significant number of rodent droppings on the sticky trap - no rodents found in the trap.
      • Corrective Action(s): Facility must be maintained in a pest-free condition. Operator to contact professional pest control services today. Pest control report is to be kept on site for review.

      • Observation (CORRECTED DURING INSPECTION): 1) Operator placed a container of raw shrimp into a prep cooler directly on top of inserts of onions and bean sprouts.
      • 2) Scoops were found stored directly inside ingredients.
      • Corrective Action(s): 1) Do not store raw foods above ready-to-eat foods. This may potentially contaminate ready-to-eat foods. Raw shrimp were relocated at the time of inspection.
      • 2) Scoops must be stored in a sanitary manner to prevent potential contamination of ingredients. Scoops were removed at the time of inspection.

      • Observation (CORRECTED DURING INSPECTION): Bleach sanitizer was too strong. Solution bleached out test strip.
      • Corrective Action(s): To properly prepare 200ppm chlorine sanitizer for sanitizing food contact surfaces, add 5mL bleach to 1L of water. Sanitizer solution was diluted to 200ppm at the time of inspection.

      • Observation (CORRECTED DURING INSPECTION): Hand soap in the men’s washroom had water added to it. Soap and water mixture was unable to properly lather.
      • Corrective Action(s): Discontinue the practice of adding water to soap. Soap must be able to lather to allow for proper handwashing. Soap was refilled at the time of inspection.

      • Observation (CORRECTED DURING INSPECTION): 1) Staff member observed preparing food with yellow rubber gloves.
      • 2) Same staff member was observed using single use gloves to clean, followed by preparing food without changing gloves.
      • Corrective Action(s): 1) Discontinue the use of rubber gloves for food handling. These are used for cleaning the facility. This may potentially contaminate foods.
      • 2) Single use gloves are intended for one time use. Once gloves become potentially contaminated or staff remove gloves from hands, gloves must be discarded.
