    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were completed during the inspection.Reprocessing sink will not be used for hand hygiene therefore written procedures for sink decontamination is not required.
    • 20. Is reusable equipment disinfected properly?
      • Improper indicator test strips were being used. Obtain proper indicator test strips & maintain high-level disinfectant (HLD) logs as required. If you are using PreEmpt CS20, you will need the 7% Accelerated Hydrogen Peroxide (Part No. AHP7) indicator test strips. In the interim, discard PreEmpt CS20 disinfectant after each use. Discussed how to maintain HLD logs and left templates on-site.
    • 20. Is reusable equipment disinfected properly?
      • The manufacturer's instructions for the intermediate level disinfectant (PreEmpt Concentrate) were not being followed. Discussed proper use. A ready-to-use disinfectant (i.e. PreEmpt RTU) that does not require dilution is preferred.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Expired sterile equipment was discarded during the inspection as per request. Please go through the entire facility and discard any expired sterile equipment.Sterile equipment that was missing sterility assurance documentation was discarded upon request.
