      • Observation: A number of the door sweeps are damaged and ineffective in preventing mouse entry such as into the boiler room -also indicated on the Orkin Pest Control report from October 25th
      • There are possible rodent entry points behind the dishwasher and the boiler room
      • Operator is using old plastic milk crates for lifting food buckets off the floor
      • Corrective Action(s): Correct ALL structural issues described in the Orkin Pest Control report -a follow-up inspection after their next visit will verify this.

      • Did not see any direct food contamination or package alteration due to rodents

      • Corrective Action(s): Clean and disinfect with bleach ALL areas with mouse droppings -check EACH MORNING with a high powered flashlight for cleaning.

      • Corrective Action(s): - Cooked rice is a potentially hazardous food. Potentially hazardous foods must be cooled rapidly according to the following parameters: Cool from 60C to 20C within a maximum of 2 hours and then from 20C to 4C within a maximum of 4 hours. Use probe thermometer to monitor cooling process. To ensure foods cool rapidly, transfer to shallow metal trays and stir. Use ice baths to further expedite cooling. Do not put hot food items directly into cooler.
      • - Cooked rice was transferred to shallow metal trays at time of the inspection. Staff member had probe thermometer to monitor remainder of cooling process.

      • Observation (CORRECTED DURING INSPECTION): Noted the stand up cooler in the deli area was not working properly to maintain cold food temperature less than 4oC. The temperature of the foods were @ 8-12oC for less than 2 hours.
      • Corrective Action(s): Foods were transferred to another working cooler. Noticed just cooked foods were moved into the cooler directly without cooling.

      • Corrective Action(s): Ensure the dispensers are working properly. The current way of using paper tower (on the counter) will wet the paper towel stack. Always monitor all paper towel dispensers and ensure they are functioning. Paper towels are restocked during the inspection.
