      • Observation (CORRECTED DURING INSPECTION):
      • - Right side of the display cooler temperature ranged between 18-21C. There is no cold air provided in this unit.
      • Corrective Action(s):
      • - There are potentially hazardous food items including tofu, soft tofu, eggs, tofu puffs, fried tofu, dried tofu stored in this area. Staff believed this area to be in the middle of defrosting stage. All items were moved to walk-in cooler. Temperature will be checked again in couple of hours. If the ambient temperature does not come back to 4C or below, all moved items will have to be discarded. Do not hide or move the food items transferred to the walk-in cooler.

      • - Total of 14 units of these rice rolls were discarded at the time of inspection. 2nd violation ticket will be issued.

      • Observation (CORRECTED DURING INSPECTION):
      • - REPEAT VIOLATION:
      • 1) Observed cooked rice rolls (from supplier) in room temperature by the cashier stand - probed at 14.4- 14.5C.
      • 2) Rice rolls in display cooler probed at 15.4C and 11.8C.
      • 3) Observed packaged/bagged mushrooms (various kind) in room temperature.
      • 4) Observed packaged/ bagged mushrooms (varioud kind) in display cooler measuring 13C - 17C.
      • Corrective Action(s):
      • - All cooked rice rolls (toal of 18 cases) were discarded as the total time spent in danger zone (4C - 60C) could not be determined.
      • - All mushrooms that were stored in warmer areas of the display cooler and in room temperature were moved back to the walk-in cooler.
      • It appears that the upper left unit of the main display cooler is not able to maintain the temperature of 4C or below. Repair and/or do not store potentially hazardous food items on this side of the cooler.
      • Violation Ticket will be issued for repeated violation of holding potentially hazardous food items in room temperature despite all past enforcement and education provided to the operator / staff.

      • - Clean hard-to-reach areas in the floor. Ensure to take out all boxes from the floor and thoroughly clean. Sanitize any sufaces contaiminated with rodent droppings with bleach solution.

      • - Packaged mushrooms, bean sprouts measured above 4C.
      • - Packaged rice rolls (prepared by supplier) measured at 20C.
      • Walk-in cooler not maintained at 4C or below (ambient temperature measured at 16C):
      • - A box of rice rolls and a box of bean sprouts measured above 4C.
      • Corrective Action(s):
      • Inner and lower shelves of the display cooler is cold. However, the upper shelf and the overstocked items are not kept at refrigerated temperature.
      • - Multiple packages of the bean sprouts & mushrooms were discarded.
      • - Rice rolls were discarded. Rice rolls should be transported after they have been fully cooled down at the supplier site. Provide the purchase receipt of the rice roll supplier to the inspector.
      • *For storage, use the small walk-in cooler to store mushrooms, bean sprouts, and rice rolls.

      • - Lower shelf is colder - other produce items are stored here. Potentially hazardous food items were moved to lower shelf at the time of inspection. Ensure to not over stock them or store them over the line where cold air is reached.

      • - A cardboard container present in the handsink basin.

      • Corrective Action(s): Immediately discard the food product. The operator must send invoice of approved supplier prior to selling the specified food product.
