      • There are thumb tacks on a cork board above a play area.- Remove thumb tacks. Ensure items are secured with non-sharp and secure device

      • 1. There is an unfinished wood board on the gate dividing the space that is visibly soiled and uncleanable- Remove the board from the gate2. There is sticky residue collecting dirt and debris on shelving- Clean sticky residue off of shelving

      • No disinfectant is present at the diapering station. A general cleaning solution available only.- Quat disinfectant is availableENSURE THE DIAPERING STATION IS SUPPLIED WITH DISINFECTANT TO BE USED BETWEEN EACH DIAPERING

      • Child safety gate does not lock properly. Staff indicated that the contractor is working on fixing the issue.Ensure gate is repaired to restrict access by clients.

      • The baby proofing cabinet locks in the kitchen are not locked.Staff closed locked the cabinet. Ensure the cabinets are always locked to protect from hazard.

      • The sleeping mats were stored on the floor horizontally on top of each other. The mats are stacked together without any separation.Ensure the mat does not touch each other when stored and protected from contamination at all times.

      • The receptacles appear to be tamper-proof however they do not have outlet covers. Ensure an outlet cover is available for all receptacles.
