    • 06. Are proper protocols being followed in regards to client care?
      • During a group toileting and diapering period, diaper changes were completed without a clean disposable liner between children, and the diapering surface was not cleaned and disinfected between each child. Hand hygiene and glove changes were not completed between children, and not all children washed their hands after toileting or diapering. The washroom environment became difficult to manage during this period, resulting in increased contact with floors and other surfaces and subsequent hand-to-mouth behaviours.These practices significantly increase the risk of fecal-oral transmission and the spread of communicable disease. Consistent and effective handwashing by both staff and children is the single most important infection prevention measure in child care settings and must occur after each diaper change or toileting event. While staff were observed making considerable effort within permitted staffing ratios, the current setup did not support consistent completion of required hand hygiene and cleaning steps.Please ensure diapering and toileting procedures are followed at all times, with particular emphasis on handwashing with soap and water for staff after each diaper change and for all children after toileting or diapering. Please ensure glove changes occur between children and are not used as a substitute for handwashing, and that diapering surfaces are cleaned and disinfected between each child.

      • Most shelves at head height in the Dinosaurs classroom now have protective covers installed; however, one shelf still lacks a protective cover. Please install a protective cover on this remaining shelf to prevent potential head injuries.

      • Protective covers are required on all shelves at head height in classrooms. Missing protective covers were observed on two shelves in the Dinosaurs classroom. Please install or replace protective covers on all shelving at head height to prevent potential injury.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • A white mark was observed on the carpet in the Bears room near the whiteboard. The stain makes the carpet appear unsanitary and detracts from overall cleanliness. Please have the carpet cleaned to remove the stain and maintain a sanitary appearance.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The boys’ washroom in the preschool wing was visibly dirty at the time of inspection. Please clean and disinfect the washroom and ensure that routine cleaning is maintained throughout the day to uphold sanitary conditions.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Single swings were observed in three classrooms. Staff indicated these are used for children with sensory needs under direct supervision and one child at a time. Swings are listed as prohibited equipment in social care facilities due to the risk of impact or entanglement injuries. While the current use is controlled and supports children’s sensory regulation, the equipment remains within the category of prohibited play items under provincial legislation. Please consult with your licensing officer to determine if a variance can be obtained for therapeutic use. If no variance is approved, remove the swings to comply with prohibited equipment requirements.

      • Some classrooms are washing toys in the classroom using a three-bucket system; however, the process observed does not ensure proper cleaning and sanitizing. Please ensure toys are washed in a plumbed-in sink with hot and cold running water. Toys must be washed in warm soapy water, rinsed, and then immersed in a bleach solution for at least two minutes of contact time before being air dried. Use test strips to verify sanitizer concentration and ensure the procedure is followed to break the chain of disease transmission.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The mat under the slide in the Turtles room is worn and no longer provides adequate shock absorption. Please replace the mat with one that provides proper cushioning and meets impact-absorbing surfacing requirements to reduce the risk of injury.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Water temperature was measured between 48 °C and 51 °C in classrooms and washrooms throughout both wings of the daycare. Hot water accessible to children in a social care facility must not exceed 49 °C, with a recommended maximum of 43 °C to further reduce scalding risk. The observed temperatures exceeded these limits and present a potential burn hazard. Please adjust or install tempering devices as needed to maintain hot water at or below 49 °C (ideally near 43 °C) at all handwash sinks accessible to children.

      • Several water bottles were not labeled with the child’s name in the OSC classroom. Labeling bottles prevents accidental sharing and potential spread of illness. Please ensure all children’s water bottles are clearly labeled with their names.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Please ensure that all sharp edges of tables and shelves in the classrooms (Honey Bunnies, Curious Kittens, Playful Penguins Funky Monkeys, and Outstanding Owls) have protective covers.2. Please ensure that all electrical outlets reachable to children have covers.3. Observed missing door lock for laundry room door. Please provide door lock for laundry room door and ensure children have no access to laundry room.4. Please remove the Christmas Tree from the top of the self in Tiny Turtle room.5. There is a small shelf placed on top of the built-in shelf in the classroom, and children are stepping onto the bottom ledge of the built-in shelf in order to reach the bins stored on the small shelf. Please place the small shelf on floor.6. Observed vitamins and bug sprays stored in the drawers accessible to children of cabinet by hand sink in Curious Kitten room. Please store them in the location inaccessible to children.

      • 1. Observed sanitizer (Bleach) solution concentration in chemical room to be at 50 ppm. To ensure optimal disinfection and safety, please verify the concentration daily and adjust as needed to maintain the required 100 ppm level.2. No litmus paper test strips are available for sanitizer (Bleach) solution at toy washing station. Please provide litmus paper test strips for sanitizer at toy cleaning station. Also provide color chart for test strips.3. Observed missing safety belt system at diaper change table in the Curious Kitten room. Please equip the diapering station with safety belt. It is recommended the diaper change table be moved to a different location so that there are no wood ledges along the diaper change pad.

      • 1. Observed holes on dry wall by white board in Playful Penguin class room.2. Observed broken floor tiles by the far corner of sensory room.Please repair above noted areas.

      • 1. Observed broken shelf at corner located by Christmas tree in Tiger in Training class room.Please repair the shelf.2. Observed toy train track attached with several duct tapes on round table in Tiny Turtles classroom.Play equipment attached by duct tape can not be cleaned and disinfected properly. Please do not use duct tape on play toys.

      • 1. Several mats were observed to be worn out, which compromises their ability to provide an adequate shock-absorbing surface for the children, posing a potential safety risk. Please ensure that above noted areas have adequate surfacing mats. Particularly in Turtles and Hippos classrooms. 2. Observed dirty water in water play table in sensory room. Please maintain the water play table clean and sanitary condition after each use.Water tables must be changed twice a day as a minimum.

      • Portable hand sink in Busy Beaver classroom was not working at the time of inspection.Please fix the portable hand sink and ensure it is capable of providing hot and cold running water with adequate pressure.

This includes refraining from using potties during lunchtime in classrooms, as this practice is not supported by licensing and does not meet the necessary standards.
