    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • The linen used to cover the client service beds in the back service area were not maintained in clean condition. Staff showed the paper liners used to cover the beds for client use.Make sure that linen used in client service bed is maintained in clean and sanitary condition, with spare set of sheets available.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • A visibly dirty, sharp razor was observed, being stored with clean products in client service area.It was discarded safely in the sharps' container during the inspection.Make sure that all the sharps are discarded right after each client use.
    • 20. Is reusable equipment disinfected properly?
      • It was observed that Pre-empt CS-20 used as high level disinfectant for semi-critical instruments was expired.The test strips for high level disinfectant were also observed to be expired.Make sure that facility has un-expired high level disinfectant and test strips available to ensure proper disinfection of semi-critical instruments.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • It was observed that there were some hair fragments in the melted wax.Make sure that cosmetic products and equipment are handled properly.Avoid double dipping, and maintain the cosmetic products and equipment in clean and sanitary manner.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • It was observed that chemical bottles were not labelled. Staff mentioned that these bottles contain Alcohol. Staff was directed to label the bottles.It was corrected during inspection.

    • 20. Is reusable equipment disinfected properly?
      • - Semi-critical equipment is not being adequately cleaned and disinfected. Pre-empt CS20 was available but was being mixed with water. Ensure semi-critical must be cleaned and disinfected using, at a minimum, a high-level disinfectant; follow manufacturer’s instructions.- High level disinfectants are not being tested and verified daily using proper test strips. Ensue the high level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.

      • -Written procedures were not available for this facility. Please ensure written procedures are developed for: Preparation of Skin, Post-Service Care and Client Care.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • -There was no skin cleanser for use prior to waxing. Ensure skin cleanser is being used.
