      • Room C175 a significantly stained ceiling tile was noted indicating a potential leak.- Ensure the cause of the leak is identified and repaired and the ceiling tile is replaced.UPDATE - Sept 11, 2025 - leak was investigated and repaired during spring. Leak was caused by water trapped in the roof over the winter months. The stained ceiling tile had not been replaced. Ceiling lighting covers in both classrooms C175 and C180 had been replaced.

    • 10. Is a properly supplied first aid kit provided and readily available?
      • The first aid kit in the out of school care room (room 210) was missing antiseptic single use wipes. - January 10, 2025 - Antiseptic wipes that had been added to the first aid kit were expired several years ago. Replace the wipes - recommend having a backup supply for replacing items as they are used and a tracking system to keep track of items that require restocking.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Room C175 a significantly stained ceiling tile was noted indicating a potential leak. Ensure the cause of the leak is identified and repaired and the ceiling tile is replaced.Both classrooms C175 and C180 had multiple ceiling light covers that were broken or missing that require replacement. - January 10, 2025 - C180 is not planned to be used for OSC and will remain a school classroom. C175 is to be used for OSC and the stained ceiling tile and 3 lights with missing covers remain outstanding.

      • The left child height handwash sink was shut off at the time of the inspection the supervisor indicated that that was done due to a water leak from the faucet.The right child height handwash sink was dripping from the base of the faucet when running.Repair both sinks.

      • Written procedures for care of the animals in room C180 is required as well as plans for how the animals will be interacting with the OSC clients. Discussed need for detailed plans with facility director and Licensing Officer during inspection.

      • Room C175 a significantly stained ceiling tile was noted indicating a potential leak. Ensure the cause of the leak is identified and repaired and the ceiling tile is replaced.Both classrooms C175 and C180 had multiple ceiling light covers that were broken or missing that require replacement.

      • Water temperature in the hand sinks measured greater than 54C and requires adjusting such that the maximum temperature at the fixture should not surpass 49C. - Facility director indicated that he would have a maintenance worker install mixing valves on the sinks right after the inspection.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • An unlabeled spray bottle was observed with the disinfectant solutions. Operator stated it was deodorizer and labeled the bottle. All chemicals shall be handled and stored properly by labeling chemical containers and stored away to prevent hazards from children.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit had expired supplies. Maintain all first aid kits with proper and non-expired supplies.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • The chlorine disinfectant solution was measuring at 0ppm. Operator was using old Clorox, a new solution was made during the inspection and remeasured at 100ppm. Ensure that all surfaces, equipment, and toys are effectively disinfected with chlorine disinfectant solutions measure at least 100ppm. Use appropriate test strips to measure the concentration for every new solution made.
