      • Observation (CORRECTED DURING INSPECTION): Pink organic buildup noted in the interior of the ice machine.
      • Corrective Action(s): Manager corrected the issue during the inspection.
      • Ice bin was emptied out for deep cleaning and sanitation of the machine.
      • Please conduct a deep cleaning of the ice machine on a more regular basis to prevent organic buildup.

      • Observation (CORRECTED DURING INSPECTION): 1. Food soil still noted on a "cleaned" cutting board that was put away with other cleaned cutting boards.
      • 2. 117 PPM DDBSA/ lactic acid based sanitizer was detected in the sanitizer container.
      • Corrective Action(s): Issues corrected at the time of inspection.
      • 1. Cutting board was placed in the dishwashing area for rewashing and sanitation. Please ensure staff do a visual check of all cleaned items to ensure that they are thoroughly cleaned and sanitized before storing away.
      • 2. Staff discarded the sanitizer solution and refilled it during the inspection. The refilled sanitizer had the correct concentration of Sink & Surface Cleaner Sanitizer.

      • Observation (CORRECTED DURING INSPECTION): 1. Under the prep table, plastic container with clean lids/utensils were stored. Rodent droppings were observed inside this container.
      • 2. Pink mildew spots observed on the inner plate (white) of the ice machine. This could result in potential contamination of ice stored in the machine.
      • Corrective Action(s): Please ensure that clean dishes/utensils are stored in sanitary manner to prevent the risk of contamination.
      • - Staff ran all the dishes/lids through high temp mechanical dishwasher during inspection.
      • - Staff started cleaning the ice machine during inspection..

      • Observation (CORRECTED DURING INSPECTION): Facility uses DDBSA/lactic acid based sanitizer for sanitizing food contact surfaces, prep tables. 0ppm concentration recorded in all 4 pails during inspection. This could result in inadequate sanitizing of the food contact surfaces. Person Incharge filled pails with new sanitizer solution. Concentration recorded within the acceptable range.
      • Corrective Action(s): Please ensure that all food contact surfaces are properly washed and sanitized.

      • Observation: Rodent droppings observed on the glue trap under shelving unit in dry storage room and on bottom shelf of prep table. Kitchen Manager stated that facility is aware of this issue and pest control company is following up on it. All gaps/holes have been sealed.
      • Corrective Action(s): Please ensure that the facility is cleaned regularly and kept free of pests.
      • - Clean the abovenoted areas.
      • - Regularly monitor for new rodent activity.
      • - Submit the pest control reports to your district EHO until further notice.

      • Observation (CORRECTED DURING INSPECTION): New formulation of QUATs solution spray bottles (i.e.: SmartPower - Sink & Surface Cleaner Sanitizer by EcoLab) measured at 0ppm. Operator QUATs test strips on-site measured at 0ppm QUATs.
      • Corrective Action(s): Switch to the old QUATs solution (i.e.: Oasis 146 Multi-QUAT Liquid Sanitizer) for sanitizing food contact surfaces. Prepare QUATs spray bottles with old QUATs formulation that measures 200ppm. Consult with EcoLab for QUATs test strips for new formulation. Do not use new QUATs formulation for food contact surfaces until it is measured to read 200ppm.
