    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • The weather stripping/door seal on the back receiving doors for the kitchen (for garbage disposal and deliveries) is observed to be in disrepair, with visible gaps at the bottom and middle seam of the doors. Sunlight can be seen from the outside through the visible door gaps. Exterior openings must be properly sealed to deter/prevent the entry of pests.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • A sanitizer bucket (QUATS) under the food preparation table at the front of the kitchen was tested and measured at a concentration of 50-100 ppm. Staff remade the sanitizer solution during the inspection, which was tested and verified at a concentration of 200 ppm. Ensure sanitizer solutions are maintained at their required concentrations (QUATS - 200 PPM) for effective sanitization of food contact surfaces and equipment. Staff must test and verify the concentration of sanitizer solutions frequently during operation and change the solutions out as needed.
    • 07. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Critical)?
      • Various canned food items in the dry food storage area were observed to have a severe dents, including but not limited to, the top/bottom seams of the cans. Operator segregated all damaged canned foods to be discarded during the inspection.Ensure that canned food items are free of damage including, but not limited to: severe dents, bulging or seam defects.
    • 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
      • A tray of Yorkshire pudding was observed to be covered with a black garbage bag in the walk-in freezer. Staff removed the garbage bag and transferred the food items into a food grade container with an appropriate lid (metal container and lid).Garbage bags are not food grade, as they contain chemicals (ie. Scented Bags) and must not be used to cover or store food. Ensure that appropriate food grade containers or covers are used to prevent cross contamination of food/food items.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • Hand sink was observed to be obstructed by a cart with steamer trays. Staff were able to move the cart out of the way when requested during the inspection. All hand sinks must be easily accessible and unobstructed at all times to ensure food staff are able to perform hand hygiene effectively.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • 1. Pest control records are not being adequately maintained. Records of pest control monitoring were reviewed on site, with the most recent record dated October 2025. Ensure that pest control records are completed monthly and available for verification/review when requested by a Public Health Inspector.2.       • The inside of the Convotherm oven at the front of the kitchen has a build-up of food debris splatter. Ensure that the above noted equipment is thoroughly cleaned and sanitized.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • Spilled dry lentils were observed in the corner behind one of the shelves in the dry food storage area. Operator swept and cleaned the area during the inspection. Food debris and spills must be attended to in a timely manner to prevent pest attraction and harborage.

    • 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
      • Chicken thighs on a metal sheet pan on storage cart in the walk-in cooler were observed to be uncovered. Operator was able to cover the chicken thighs with saran wrap during the inspection as requested. Ensure that stored food items are protected with adequate lids or covers to prevent introduction of external contaminants.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • The facility utilizes QUATS sanitizer in the chemical auto feeder for the 3-compartment sink. The facility does not currently have the appropriate test strips to verify the sanitizer concentration when it is dispensed. In the interim, PHI advised to utilize chlorine (bleach) sanitizer solution in the 3-compartment sink when completing manual dishwashing, as the facility has appropriate test strips for chlorine. Ensure that new QUATS test strips are obtained to accurately test and verify the concentration during manual dishwashing at this facility.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Pest control records are not being adequately maintained. New AHS pest control record template was provided to the operator during the inspection. Even when pests are not evident, a pest monitoring program must be in place. Please ensure to complete pest control records monthly and keep them on site for verification by a Public Health Inspector. **Note: There were no signs pest activity observed during the inspection
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • The following food equipment was not being stored correctly:1. Scoop for bulk container of oats was observed to be stored on top of the container2. Ice machine scoops were observed to be stored directly on dry storage shelf (not in an appropriate container) in front of the machineOperator removed the scoops to be washed and sanitized during the inspection.Please ensure that scoops used for dispensing bulk foods are stored appropriately (ie. in a storage container) to prevent cross-contamination of the bulk food supply.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • A sanitizer bucket containing bleach solution was tested and measured at a concentration exceeding 200 ppm (test strip was bleached out). PHI instructed staff to remake the solution which was tested at a concentration of 100 ppm. Ensure that bleach sanitizer solutions are maintained at the required concentration of 100 ppm.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • Expired chemical testing equipment was observed:1. Chlorine test strips (EXP: 03/2022)2. QUATS test strips (EXP: Oct/2025)PHI left a few chlorine test strips during the inspection.Ensure that new test strips are obtained to accurately test and verify the concentration each time a new sanitizer solution is made during food handling and manual dishwashing at this facility.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Various plastic spatulas were observed to be in disrepair. The spatulas were discarded during the inspection. Ensure that all food equipment and utensils are maintained in a good working condition for proper cleaning and sanitization.

    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • A frozen beef brisket was observed to be thawing on a large sheet pan at room temperature. PHI had staff move the sheet pan into the refrigeration cooler. Ensure that proper thawing methods for frozen foods are being followed: in the refrigerator, immersion in cold water, or in the microwave to prevent bacterial growth/contamination.

      • A spray bottle containing sanitizer solution was measured at 0 ppm. The operator was using chlorine solution prepared in a designated bucket with cleaning cloth left in the solution. The operator emptied the spray bottle during inspection.Ensure that staff regularly check the concentration of sanitizer solutions using the available test strips.

    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • A stainless steel container with fried rice was stored on a grill as hot holding. The temperature was measured at 50 degrees Celsius. Operator moved the food item into the walk in cooler during inspection.Ensure that all potentially hazardous foods are kept in hot holding at or above 60 degrees Celsius, or in refrigeration at or under 4 degrees Celsius.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • Test strips to monitor concentration of chemical sanitizer (Chlorine) for surface sanitization were not available on site. Ensure that working monitoring device is available onsite to ensure adequate concentration of sanitizer solutions. Inspector left a few test strips.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • No records of pest control were available on site.Ensure that the pest control checklist is completed monthly and kept on site. Template was provided to the operator by email.
