- At the time of inspection, the outdoor play/walkway area was not maintained in a safe condition. Ice accumulation and a large crack/uneven walking surface were observed in a child-accessible area.Outdoor premises used by a day care facility must be maintained in such a state of repair that they do not create a hazard to the safe and sanitary operation of the facility, and persons must be adequately protected from hazards. Stairways and walkways must be maintained free from snow, ice, and other hazards. If not corrected, these conditions may continue to create slip and trip hazards, including during freeze-thaw periods. Video footage later provided by the operator was consistent with the ongoing hazard potential of the affected area.Please correct the outdoor slip and trip hazards. Please ensure ice accumulation is removed or effectively controlled, and please repair or otherwise make safe the cracked/uneven walking surface in child-accessible areas.

- Rust was observed on the partition between the toilet stalls in the children's washroom. Please repair or refinish the partition surface to ensure it is smooth, cleanable, and maintained in good condition.

- The sewage-like odour associated with the children’s washroom floor drain was significantly decreased at the time of reinspection. To date, no symptoms associated with the odour have been reported to AHS.The public health risk appears reduced based on conditions observed at the time of reinspection. However, the odour concern remains relevant because sewage-like odours may affect indoor air quality if the odour becomes strong, persistent, or associated with symptoms.Please provide written confirmation outlining what assessment was completed, the suspected source of the odour, and what ongoing maintenance measures are required to prevent intensification of the odour. If the odour becomes strong or persistent, spreads beyond the washroom, or if staff/children report symptoms such as headache, nausea, eye/throat irritation, dizziness, or respiratory discomfort, the operator is to notify AHS for further assessment.

- Several shelving units located at approximately head height were observed to have sharp edges. Please repair, cover, or otherwise address these sharp edges to reduce the risk of injury to children.

- The garbage bin in the washroom was overflowing with paper towels, and some waste had fallen onto the floor. Please ensure garbage containers are emptied regularly and maintained in a sanitary condition to prevent overflow and maintain cleanliness.

- 1. Toilet seat trainers were observed stored directly on the bathroom floor and were visibly dirty. Please store toilet seat trainers in a clean manner off the floor and ensure they are cleaned and sanitized between uses.2. The diaper change table surface/pad was observed stored directly on the bathroom floor. Please store diaper changing equipment in a clean location off the floor to prevent contamination and maintain sanitary conditions.3. Wiping cloths were observed stored wet and hanging on a spray bottle at the time of inspection. Storing wiping cloths in this manner may allow microbial growth. In childcare settings, sanitizer buckets for storing wiping cloths are not recommended. Please ensure wiping cloths are used as single-use cloths and placed for laundering after use. Alternatively, ensure cloths are replaced with clean cloths at least every two hours.

- A strong sewage-like odour was detected in the children’s washroom originating from the floor drain. Sewer gases can pose an indoor air quality concern and may cause irritation or illness, particularly in young children. Please investigate and correct the source of the odour to ensure the washroom environment remains safe and sanitary.

- Hot water at the children’s handwashing sink was measured at 60°C at the time of inspection. This temperature presents a significant scald risk to children and is not acceptable. Water at handwashing sinks used by children must not exceed 49°C. Please immediately adjust the hot water supply or install an appropriate temperature control device to ensure water temperatures at children’s handwashing sinks do not exceed 49°C at any time, the recommended temperature is 43C.
