      • 1 dead mouse observed near the play mattresses. Mouse droppings observed. Ensure adequate cleaning and disinfection is done. Pest control specialist follow up will need to be completed.

      • Weatherstripping in disrepair (door leading to the outside playground). Big gaps noted between door and floor. Ensure weatherstripping is fixed to prevent pest entry.

      • Opening and closing cleaning check list observed to be filled out. The visit was started around 9:30 AM. Ensure cleaning records are maintained as cleaning is completed and not prior to task being completed. Cleaning observed to be needed at the time of the visit.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Preschool Room - Toilet bowl cleaner and goo gone chemicals were observed on the bathroom ledge. Please store these items in a location that is not accessible to the children.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Infant room - there were toys and other items present in the cribs.Infant room - there was a string on one crib tied to each side at the top of the crib. A toy was hanging from the string as a mobile.Please remove all extraneous items form the cribs.
    • 06. Are proper protocols being followed in regards to client care?
      • Diapering supplies and food, including formula were located on the same shelf / area. Please ensure these items are labelled in their own designated area.

      • Infant room - water staining was observed on the ceiling in the infant room. Please ensure that there are no leaks and replace the damage ceiling tiles.

      • OSC Room - The legs of the air hocky table were not secure from wobbling. Please repair or remove the item. This deficiency was outstanding from previous inspection.

      • Infant room - toys in the infant room were being washed in the hand sink used for diapering. Please ensure that toys washing is occuring in the kitchen sinks outside f meal prep and kitchen clean up. Other sinks may be utilized provided they are not used for diapering.Infant room - the cover for the infant swing was dirty. Please ensure the cover is being removed and washed at minimum weekly.Toddler Room - a fabric rocking horse was observed. This material cannot be adequately cleaned. Please remove from use.
