- Observation (CORRECTED DURING INSPECTION): Probed PHFs stored on the insert level had an internal temperature of 7C. Ambient temperature of the prep cooler was also 7C.
- Corrective Action(s): Insert items that cannot be used within 4 hours must be discarded. Until the prep cooler is serviced and able to maintain 4C or colder, store only a small amount for immediate use. Keep the rest in the other working coolers (stand up cooler or walk in cooler) or prep when needed.

- Observation (CORRECTED DURING INSPECTION): Potentially hazardous foods (chopped cucumbers, chopped red onions, sliced tomatoes, shredded lettuce, and shredded cheese) stored on the insert level all measured above 4C. Internal temperature probed to be 14.9C. Inserts were filled at the start of the day at 10AM, time of inspection was approximately 6 hours later.
- Corrective Action(s): All potentially hazardous foods named must be discarded immediately. Only pickled items and recently made humus and tzaziki sauce may be kept here until the end of the day.
- Do not store any PHF toppings in the insert level until the air temperature for the whole cooler is 4C or colder.
