The program staff indicated that ointment or creams for diapering are applied using the staff fingers not an applicator.If applying ointment on a child during diapering, use a single-use (disposable) applicator for the ointments or creams to prevent cross contamination.Update Jan 201, 2026: Facility operator demonstrated the procedure followed for diapering. Program staff uses their hand with clean gloves for scooping up the ointment or creams from the main container, then apply it to the child. - Provide disposable applicators for this purpose to eliminate potential cross contamination.

- The belt restrain on the diaper change table was broken on one end. Adequate restraints such as safety belts or other approved device shall be provided for every diaper changing surface that is elevated above floor level.

- Chlorine test strips for measuring concentration above 200ppm was not available at the facility. Facility staff were using test strips that only measure chlorine solution up to 200ppm. Provide the appropriate test strip for measuring concentration of chlorine disinfectant up to 1000ppm.

- Surfaces of a play equipment in the preschool room was unclean. Bins used for storing natural materials for play was unclean.Surfaces were cleaned thoroughly during inspection. Ensure equipment surfaces around the facility are cleaned on a routine basis.

- Crayons broken into small pieces were found in containers near the children's lunch table and cabinet near the front entrance. Remove the small broken crayons and to prevent choking hazards.

- The corners of the diapering table were dirty with accumulated debris.Facility operator cleaned the diapering table. Ensure that surfaces and corners of the diapering table are cleaned and disinfected thoroughly after each use.

- A ceiling light cover above the children's eating table was cracked and inadequately installed. Replace the light cover and ensure that all light fixtures are adequately covered.

- Air vents in the childrens washroom and above the diapering station were observed dusty.Clean the above-mentioned areas and ensure they are maintained in a clean and sanitary condition.

- The water tap in the children's washroom temperature was measured at 55.3C.Facility Operator stated that children are supervised during hand washing. Ensure that any tap accessible to children has a maximum temperature of 54C, ideally 49C.

- The mattress in the crib was too big for dimensions of the frame. The mattress was flush against all four sides of the crib. There should be no more than 3 cm (1.2 in) of space between the mattress and the bed frame when the mattress is pushed flush to one corner of the crib.
