      • Sterility assurance documentation for critical piercing equipment was not retained.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • - No documentation of sterilizer monitoring (physical, chemical, or biological) was not done from after February 26, 2023, until October 13, 2023.- Packages of sterilized equipment were not labeled with load numbers. As such, they do not have adequate sterility assurance.- Of the records that were available, physical monitoring had a temperature of 135C for 60 minutes, however the cycle time was recorded, and not the sterilization time.- Type 5 chemical indicators were used; however, results were not documented.Last satisfactory BI was completed on February 17, 2023. BIs have been expired since September 30, 2023. Operator indicated that both test and control BIs exhibited no growth, and may mean that the BIs could be giving false negative results. During the inspection, an unprocessed BI was cracked, then placed in the incubator, which was being maintained at 57C as required. Operator will notify PHI regarding the results.Corrective Actions:Physical monitoring: Since the autoclave in use does not have instructions for sterilization time ensure a standard sterilization temperature of 134C is maintained for 10 minutes, or 132C for 15 minutes. This needs to be visually confirmed using the analog temperature/pressure gauge.- Ensure one type 5 chemical indicator is placed inside a package for every cycle.- Acquire new BIs and ensure you obtain satisfactory results. Test vial - growth; control vial - no growth.- Once satisfactory physical, chemical and biological monitoring has been completed, ensure all packages without a load number are repackaged, reprocessed, and relabeled.

      • Written procedures required for:- Decontamination of the sink in the reprocessing are when switching between hand hygiene and equipment reprocessing.- actions to be taken for failed sterilization cycles.

      • Sharps container in the piercing room was filled beyond capacity. Waste sharps were sticking out of the container.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The reprocessing area does not allow equipment to be adequately cleaned. The single compartment sink does not allow equipment to be separately rinsed.In the absence of a second compartment, please acquire a container that will be used to rinse reusable piercing equipment.Procedures for cleaning:The container will be filled with water and placed to the side. Cleaning will occur under water in the sink, then rinsed under the surface of the water in the container to minimize splashing and aerosolization of contaminated water.Equipment should be dried with a clean towel or allowed to air dry.
