- Observation: Signs of mouse activity noted. Multiple snap traps in back area of store, not set and observed with old food on them. Poison pellets noted all over the back area of store floor, especially by the produce section. Some have disintegrated with moisture, other areas poison in tact. This poses a significant risk to exposure to food product. Discussed with operator.
- Corrective Action(s): Conact pest control and have technician take lead in control efforts. Only Certified Pest Control technician should be applying any related pesticides. Pest Control Technician is needed to advise and take necessary actions to control/eliminate mouse problems.
- Continue to clean and organize store to reduce food opportunities for rodents.
- Send copies of PCO recommendations to this office.
- Immediately remove any poisons from facility and clean exposed areas as discussed.

- Observation: A) Produce area observed dirty and disorganized. Food containers stored under handsink. Work space limited. Use of mobile dollies for prep work of vegetables. Cutting boards are dirty and not adequately cleaned on regular basis. Cutting boards stained and discoloured.
- B) Floor of deli walk-in cooler is wet and dirty.
- C) shelves of walk-in cooler, walk-in Freezers in back area of store made of wood (Planks and plywood) and are not easily cleanable. Also not being cleaned properly. They are dirty (mould?). Accumulations of food debris observed building up on much of the wood shelving in most of these units.
- Corrective Action(s): A) This area must be cleaned and organized. Invest in more cutting boards such that staff are not required to use the same one all day. Cleaning, sanitizing and periodic bleaching of cutting boards needed to help cue staff when work surfaces get dirty and require cleaning. Must be stored in a manner that minimizes potential for contamination.
- B) Clean and keep all food off the floor.
- C) Plan for shelving replacement to surface that can be easily cleaned and maintained by staff as discussed.

- Observation: Bottom shelf in produce walk-in cooler (brocoli shelf?) broken and dirty. This is contributing to the rest of the cooler floor in this area to be dirty.
- Corrective Action(s): Replace shelf (with metal) and clean up the floor in this cooler. Phase out wooden shelves in produce cooler with metal as needed. This will be reviewed at next inspetion.
