      • The hand soap dispenser at the handwashing sink was not working.Soap dispenser was adjusted and was then dispensing soap.Ensure soap dispenser is operational at all times.

      • The antiseptic wipes in the kit were expired.Replace expired products.

Chlorine test strips are present but expired 02/25.Obtain current test strips.Test strips must be used to verify sanitizer concentration each time a solution is prepared, and if solutions are used over multiple days, each day prior to use.2. Two water bottles were not labeled with the child's name.Ensure all water bottles are labeled with the child's name.

      • There are several cracks present in the flooring.Ensure the flooring is maintained so it is smooth, impervious to moisture, and easy to clean.Small cracks may be sealed; larger areas of damage may need flooring replacement.

      • The fridge measured 11C.It was reported that some parents include ice packs in their child's lunches and some do not.High risk food items in lunches as well as milk/formula, must be maintained at 4C or less.Options include:- repair or replace the fridge so it is capable of maintaining a temperature of 4C or less;- remind parents to include ice packs with their child's lunch if high risk food items are included- only allow low risk food items to be included in children's lunches.

      • A garbage can is not available in the immediate vicinity of the staff handwashing sink.Ensure an easily accessible garbage can is present within close proximity to the staff handwashing sink.

      • There are several cracks present in the flooring.Ensure the flooring is maintained so it is smooth, impervious to moisture, and easy to clean.Small cracks may be sealed; larger areas of damage may need flooring replacement.

      • Children were accessing a play area in the ACFA building. The play area was an open room with a stage and some chairs. Toys were present that are also used for a mom/tot program on Thursdays.During the outbreak it is strongly recommended that the shared play space not be used. If the space is used, it must be thoroughly cleaned and disinfected after use. Do not use shared toys with the mom/tot program for the duration of the outbreak. Any toys already used must be thoroughly cleaned and disinfected.

      • The fridge used to store children's lunches measured 9.1C and was on the second coldest setting.Fridge was placed on the coldest setting during inspection.A temperature monitoring sheet was present but was not consistently filled out. Dates were present but not temperatures.Ensure the temperature of the fridge is recorded every day it is used. It is recommended to take the fridge temperature early in the morning to ensure it is working prior to use.Monitor the fridge to ensure it is capable of maintaining a temperature of 4C or less.A thermometer was present in the fridge.

      • The fridge used to store children's lunches measured 13.2C and was on the warmest setting.Fridge was placed on the coldest setting during inspection.Monitor the fridge to ensure it is capable of maintaining a temperature of 4C or less.A thermometer was present in the fridge.

      • The spray bottle for bleach was new and not labeled.Ensure all bottles/containers containing chemicals are labeled to indicate contents.
