      • A cardboard with exposed nails was found on a shelf in headstart room.- The operator removed the cardboard during the inspection.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Water damage was noted on the ceiling tiles in wolf room. - Find the source of the leak and replace the tiles.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit in headstart room was low on bandages. - Restock the bandages.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1) Toy bins throughout the facility had dust accumulation.- Clean the bins.2) The black shelf was dusty in headstart room. - Clean the shelf.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1) The bean bag in the wolf room was dirty. - Wash or steam clean. 2) Xylophone toy in Bears room had visible dust and debris accumulated. - Clean the toy.

    • 10. Is a properly supplied first aid kit provided and readily available?
      • - The first aid kits were not stocked as required. A list of required items was provided via email. Ensure all first aid kits are updated with the necessary items.
    • 13. Is this facility properly heated to ensure client comfort (Critical)?
      • - The temperature in two of the rooms was observed to be 18.2°C and 19.4°C.*Staff increased the heat, and the temperature began to rise. Ensure that a minimum temperature of 20°C is maintained in all rooms.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1) Dirt was observed on sensory toys in Room 3. Staff segregated the toys for cleaning. Ensure that sanitization and cleaning practices are performed regularly. Any toys that are visibly dirty should be removed and not placed back in the room until proper sanitation is completed.2) - Soft balls (red and yellow) were in disrepair. The non-absorbent top layer was peeled, and the surface was rough and absorbent, preventing proper sanitation. The staff discarded the balls.

      • In Room 1, there were three holes in the wall behind the door, and a ripped duct tape was observed.*Repair the wall. Ensure all surfaces are smooth, non-absorbent, easily cleanable, and free from damage.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Acrylic paint bottle not suitable for children under 3+ was observed in Eagle room.-Please remove the indicated paint bottle and ensure all craft materials in all daycare rooms are age appropriate, labelled with an "AP" stamp or labelled as non-toxic.
    • 06. Are proper protocols being followed in regards to client care?
      • Dirt was observed under the diapering mats.-Please clean and sanitizer diapering surfaces including under the mats immediately after each diapering process to ensure the surfaces are maintained in sanitary condition at all times.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Damaged window panel and peeling paint were observed in Bird room.-Please repair and ensure good repair.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There were no hand drying paper towels available at the diapering' sink.-Please provide and ensure paper towels are readily available at the indicated sink at all times to ensure proper hand washing process.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit was not available in Bird room at the time of the inspection.-Please provide well-stocked first aid kit in the indicated room and ensure it is readily available at all times.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Dirty carpets were observed in all daycare rooms-Please clean all carpets and ensure they are vacuumed daily and steam cleaned regularly and when needed.2. The sanitizer concentration (bleach solution used to sanitize surface and equipment) in Wolf room was inadequate, tested between 10-50ppm.-Please prepare and use 100-200ppm bleach solution to clean and sanitize surfaces, ensure to daily monitor the bleach concentration with appropriate test strip.

      • Dirty toys and play equipment were observed in Bird and Eagle rooms.-Please clean and sanitize all toys and play equipment as required.
