      • Push pins observed on several classrooms' walls, including baby room, preschool and facility's corridor.-Please remove push pins from these walls. Use safer methods or protective materials to display items on walls to prevent potential injury hazards. In addition, where staple pins are used, please cover with adhesive tape to prevent children's access to hazardous material such as pins.

      • No paper towels available in the corridor's handwashing sink.-Please provide paper towels in the indicated sink area to support proper handwashing practices.In addition, provide paper towels in suitable disperser so that the paper towels can be accessible one at a time to prevent bulk contamination.

      • Bottles of children milk kept in baby classroom at room temperature.-Please ensure all children 's bottles of milk are labeled with their names and stored in the refrigerator (maintained at or below 4'C) until needed.

      • Torn diaper mats and dirty diaper table observed in the facility's diapering areas.-Please relace the diaper mats to ensure smooth, non-absorbent and easy to clean surfaces. In addition, clean and sanitize the diaper table after each use to ensure sanitary conditions.

      • 1. Dirty vents observed in several classrooms including baby's and 2-3 year olds.-Please clean all dirty vents to ensure sanitary conditions.2. Dirty water bottles container was observed in the preschool classroom.-Please clean the indicated container to maintain sanitary conditions.3. Rough and peeling paint observed in several areas, including on 4-5 years olds classroom's walls, walls adjacent to diapering stations and on window panel in the library room.-Please smoothen and repaint these areas to ensure easy to clean surfaces and good repair.

      • Blankets used by children were stored together and in direct contact with one another.-Please label and store separately blankets that are being used by children.Blankets used by children must be stored individually, such as in separate bags or designated compartments, to ensure they do not come into contact with one another.

      • A cardboard with exposed nails was found on a shelf in headstart room.- The operator removed the cardboard during the inspection.

      • Water damage was noted on the ceiling tiles in wolf room. - Find the source of the leak and replace the tiles.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit in headstart room was low on bandages. - Restock the bandages.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1) Toy bins throughout the facility had dust accumulation.- Clean the bins.2) The black shelf was dusty in headstart room. - Clean the shelf.

      • 1) The bean bag in the wolf room was dirty. - Wash or steam clean. 2) Xylophone toy in Bears room had visible dust and debris accumulated. - Clean the toy.

      • - The first aid kits were not stocked as required. A list of required items was provided via email. Ensure all first aid kits are updated with the necessary items.
    • 13. Is this facility properly heated to ensure client comfort (Critical)?
      • - The temperature in two of the rooms was observed to be 18.2°C and 19.4°C.*Staff increased the heat, and the temperature began to rise. Ensure that a minimum temperature of 20°C is maintained in all rooms.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1) Dirt was observed on sensory toys in Room 3. Staff segregated the toys for cleaning. Ensure that sanitization and cleaning practices are performed regularly. Any toys that are visibly dirty should be removed and not placed back in the room until proper sanitation is completed.2) - Soft balls (red and yellow) were in disrepair. The non-absorbent top layer was peeled, and the surface was rough and absorbent, preventing proper sanitation. The staff discarded the balls.

      • In Room 1, there were three holes in the wall behind the door, and a ripped duct tape was observed.*Repair the wall. Ensure all surfaces are smooth, non-absorbent, easily cleanable, and free from damage.

      • Acrylic paint bottle not suitable for children under 3+ was observed in Eagle room.-Please remove the indicated paint bottle and ensure all craft materials in all daycare rooms are age appropriate, labelled with an "AP" stamp or labelled as non-toxic.
    • 06. Are proper protocols being followed in regards to client care?
      • Dirt was observed under the diapering mats.-Please clean and sanitizer diapering surfaces including under the mats immediately after each diapering process to ensure the surfaces are maintained in sanitary condition at all times.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Damaged window panel and peeling paint were observed in Bird room.-Please repair and ensure good repair.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There were no hand drying paper towels available at the diapering' sink.-Please provide and ensure paper towels are readily available at the indicated sink at all times to ensure proper hand washing process.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit was not available in Bird room at the time of the inspection.-Please provide well-stocked first aid kit in the indicated room and ensure it is readily available at all times.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Dirty carpets were observed in all daycare rooms-Please clean all carpets and ensure they are vacuumed daily and steam cleaned regularly and when needed.2. The sanitizer concentration (bleach solution used to sanitize surface and equipment) in Wolf room was inadequate, tested between 10-50ppm.-Please prepare and use 100-200ppm bleach solution to clean and sanitize surfaces, ensure to daily monitor the bleach concentration with appropriate test strip.

      • Dirty toys and play equipment were observed in Bird and Eagle rooms.-Please clean and sanitize all toys and play equipment as required.
