      • Staples were used to hold up a sensory cloth located in the toddler 1 room and within reach of children.Please do not use staples to hang up any items within reach of children. Please remove the sensory cloth completely or find a safer alternative (3M hooks).
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1) Staples were used throughout the facility.Please ensure that all staples are kept out of reach of infants and toddlers, and that any exposed staples are securely taped down to prevent them from falling and posing a choking hazard.2) A safety bumper was missing from the pillar corner in the indoor play area.Please install a safety bumper to prevent accidental bumps or injuries.

      • The surfaces underneath the diaper changing mats throughout the facility were soiled in dirt and debris.Please clean and sanitize the indicated surfaces.

      • The window ledge located in the preschool 1 room was repaired with mud but not sealed.Please seal the window ledge by painting it to create a smooth, easily cleanable surface.

      • The first aid kits were equipped with expired antiseptic wipes and were missing CPR masks and tweezers.Please ensure the first aid kits are equipped with antiseptic wipes within their expiration dates, CPR masks, and tweezers.

      • 1) The black garbage bin located in the toddler 1 room was covered in stains and caked on sauces.Please clean and sanitize the garbage bin.2) The sides and handles of the highchairs located in the infant room were covered in stains and leftover food debris.Please clean and sanitize all the highchairs located in the infant room.3) Loose debris and particles were trapped in the blue rug located in the toddler 2 room.Please deep clean the area rug.4) The plastic play kitchen located in the preschool 1 room was covered in dust and debris.Please deep clean and sanitize the play kitchen.

      • Diapering area is very close to the food prep counter in the infant room; Diapering and food prep area share the same hand sink. Gloves are not changed between tasks.Relocate the diaper change table to the washroom and remove all storage items from the washroom.Ensure staff and children's hands are washed in the washroom sink after diapering.Ensure that gloves are discarded between tasks and hands are washed in between each task.Develop a glove use policy and have an education session on proper glove use.

      • 1) Paint is chipping by the window ledges in several rooms. Please sand and repaint these areas. 2) The indoor gym area has tape holding down the foam corners.
