      • PHI received a complaint regarding coffee cup lids accidently bumped onto the floor and were picked up and placed on the countertop, near the drive thru window. Now you have contaminated coffee cup lids and a countertop.This complaint was discussed with the shift supervisor.The shift supervisor reported that McDonald's policy is to have discarded the coffee cup lids that fell on the floor. She will discuss with the owner, as well as reaffirm this policy with facility staff.

      • At the rear main receiving door, the bottom door sweep is quite worn under the door handle side of the door.Corrective action - please replace this door sweep to potentially keep any insects and rodents out of your facility.
