- Chlorine test strips were expired and Quats test strips not available. Ensure valid test strips are available at all times to confirm the sanitizer concentrations for effective sanitization.**Please obtain new test strips for both the dishwasher and the contact surfaces sanitizers.

- The ice machine with ice in it had a door missing at the back. Ensure the door is repaired or replaced to keep ice stored at the corrected temperature and protected from outside contaminates. **Please repair and replace the ice machine door

- October 26, 2023:Acoustic style ceiling tiles had been used to replace the missing ceiling tiles from the west side of the kitchen.Replace the acoustic style ceiling tiles with ceiling tiles that are smooth, non-absorbent to moisture, and easy to clean.October 3, 2023:There were missing tiles on the west side wall of the kitchen (close to ice machines), exposing the drywall underneath.Repair the area where the tiles are missing. Ensure the area is smooth, non-absorbent to moisture, easy to clean, and constructed from materials that are intended for the purpose.

- The concentration of quat based sanitizer prepared was undetectable. Concentrated bottle hooked up to dispenser was changed out for a new bottle, and then concentration measured 200 ppm.Ensure that the concentration of quat based sanitizer solution is at the acceptable level for sanitizing food contact surfaces (200 ppm).

- Residential chemical pesticide spray bottles were present in the kitchen.Remove any chemical pesticide bottles from the facility. All pesticide application in the facility must be done by professional pest control operators.

- The soap dispenser for the kitchen hand wash sink was empty. A counter top bottle of soap was placed at the kitchen hand wash sink during inspection. Ensure that all hand wash supplies are available at all hand wash sinks at all times.
