A hole was observed at the lower corner beneath the rear screen door, which may allow pests to enter the facility. - Ensure the hole is repaired and sealed in a manner that prevents the entry of pests.2. Pest control records were not available for review at the time of inspection. - Ensure pest control measures and monitoring activities are implemented and documented at least monthly.

      • An open bag of dry food (raw rice) was observed being stored directly on the floor.During the inspection, staff transferred the rice into a plastic lidded bin and removed it from the floor. - Ensure all dry food items are stored in rodent-proof containers and kept off the floor to prevent potential contamination and pest attraction.

      • Several utensils were noted being stored inside the hand sink in the kitchen. And the sink was not draining properly. - Staff corrected during inspection. Refrain from storing utensils in the hand sink. Keep the sink in good working order at all times.
