    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED EIGHT RODENT DROPPINGS ON FLOOR OF BASEMENT, ONE DEAD MOUSE ON SNAP TRAP IN BASEMENT AND SEVERAL DEAD WATER BUGS ON FLOOR OF BASEMENT. MUST CLEAN ALL AREAS. RODENT PROOF SIDE OF TAVERN ENTRANCE DOOR. PEST CONTROL REPORT MENTIONS ALL OF THE ABOVE AND HOLES THAT NEED SEALING IN THREE BASEMENTS.

      • When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. DOOR TO MENS WASHROOM IN TAVERN IS A HALF DOOR. FULL DOOR WAS REMOVED BY OWNER. OTHER PATRONS CAN SEE INSIDE AS THEY WALK BY IF WASHROOM IS IN USE. INSTRUCTED THAT A FULL DOOR MUST BE PUT IN PLACE OF HALF DOOR AND BE COMPLETELY ENCLOSED PER CHICAGO MUNICIPAL CODE P. 87.
    • 27. TOILET ROOMS ENCLOSED CLEAN, PROVIDED WITH HAND CLEANSER, SANITARY HAND DRYING DEVICES AND PROPER WASTE RECEPTACLES
      • In all food establishments, toilet facilities shall be kept clean and in good repair and shall include an adequate supply of hot and cold or tempered water, soap, and approved sanitary towels or other approved hand-drying devices. NO HANDSOAP FOR HANDWASHING IN MENS WASHROOM FOR PATRONS; MUST PROVIDE PER CHICAGO MUNICIPAL CODE.

      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST FLUSH/CLEAN SODA, JUICE DISPENSER LINES AT BAR. CLEAN INTERIOR BOTTOM OF COOLER BEHIND BAR.

      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOOR UNDER BAR COUNTER. SEAL HOLES IN FLOOR OF BASEMENT UNDERNEATH TAVREN, PROVIDE A PROPER SUB PUMP COVER AND PROVIDE FLOOR DRAIN COVERS WHERE MISSING IN BASEMENT.
    • 35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
      • The walls and ceilings shall be in good repair and easily cleaned. CLEAN VENT FAN IN MENS WASHROOM TO REMOVE DUST.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • REPAIR BOTH MEN AND WOMENS VENT FANS IN WASHROOMS THAT ARE NOT WORKING.
    • 41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
      • ORGANIZE BASEMENT, REMOVE UNNECESSARY ITEMS TO AVOID CLUTTER.
