    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures were available on site for cleansing skin, reprocessing sink decontamination, and accidental skin cuts and punctures. Ensure that the written procedure templates are completed and kept on site at all times. An operator of a Personal Services Establishment must be able to easily access these documents for review/revision and when requested by a Public Health Inspector.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility documentation of sterile critical equipment (SkinPen single-use microneedling cartridge) is not being kept with client records. Inspector advised operator to attach each cartridge backing sticker with all sterility assurance information to client records going forward. Ensure that sterility assurance documentation from all critical equipment purchased as sterile is kept with client records.
    • 20. Is reusable equipment disinfected properly?
      • Testing results for high-level disinfectant (HLD5 - MDL 103056) are inadequately maintained. Operator has a physical blank AHS disinfectant log sheet template in the facility. Test results of high-level disinfectants must be monitored, recorded and maintained each time it is used to ensure that the minimum effective concentration for disinfection is verified.

    • 20. Is reusable equipment disinfected properly?
      • The following items were noted to be expired: 1. PREempt HDL5 2% Accelerated Hydrogen Peroxide test strips 2. PREempt HDL5 2% Accelerated Hydrogen Peroxide disinfectant (MDL: 103056) expired in July 20233. Myo-Therx Genie Plus wipes (DIN: 02251868): expired in March 2023Operator was able to discard expired products during the inspection. Operator will be using CAVI Wipes (DIN: 02242209) instead of the Genie Wipes to wipe down procedure bed surface. Please ensure disinfectant solutions and wipes are not expired when in use for effective disinfection/sterilization of surfaces/items and to have un-expired test strips available for accurate concentration testing/monitoring of chemical disinfectants.
