- Observation(s): Chipped paint on wall in dining area, as well as in PIC's room adjacent to living room; damage to wall at top of stairs where a hand sanitizer unit used to be.

- Observation(s): The process in place for reporting incidents is ineffective. It was noted during the inspection a minor incident report on aggressive behaviour was on record for a person in care. In discussion with the manager, the facility does not have any behaviour care plans in place and this was an aggressive incident not reported to Licensing.

- Observation(s): The system for ensuring compliance with safe storage of cleaning agents is ineffective. A cleaning solution was found unlocked in a bathroom cupboard. The cleaning solution was removed and placed in a secure area during the inspection. This item was corrected during the inspection.

- Observation(s): The system for ensuring compliance with reviewing and modifying the care plan is ineffective. The nutritional care plan did not include specific meal preferences requested by the person in care.

- Observation(s): Staffing spread sheet made accessible by computer access to the Licensing Officer and one staff who was hired in January 2016 does not have tuberculosis screening checked off as obtained.

- Observation(s): Staffing spread sheet made accessible by computer access to the Licensing Officer and one staff who was hired in January 2016 does not have the criminal record check marked off as obtained along with expiry date.

- Observation(s): Carpet on stairway leading down to the lower level recreation room noted to be stained and soiled in several areas. Lock underneath sink in upstairs kitchen is not working and hazardous dish washer detergent is accessible to persons in care. This remains outstanding from past inspection. There is a lawnmower stored on the patio of the basement accessible to persons in care. A person in cares medication was being stored unlocked in the main kitchen fridge and accessible to other persons in care. The medication was removed from the fridge by the manager and locked in the medication cupboard during the inspection. This item was corrected during the inspection.

- Observation(s): The majority of the health and oral care plans have not been reviewed for over 1 year.

- Observation(s): The posted program calendar did not include all of the activities offered in house and had several days where no programs were being offered.
- Provide a program of activities, without charge, that is suitable to the needs of persons in care (Does not apply to Hospice); 55(1)(a)(i)

- Observation(s): There are several days in the calendar that do not include activities and several activities offered during this month were not included on the activity calendar - example given - Velentines day activity was missing from the calendar.

- Observation(s): As per inspection report - there were several days where there were no planned programs and program calendar was not kept up to date with the programs offered in house.

- Observation(s): Menu plan did not always contain two food groups for each of the two snacks being provided by the facility.

- Observation(s): There were several missed entries for monitoring fridge temperatures noted on the temperature log. Unable to locate the policy on outbreak prevention and control. The facility also did not have any posters up in regards to influenza vaccinations for visitors or masks left out for visitors. There are masks on site, but they are not kept out for visitors to have access to.

- Observation(s): One of the emergency exits was partially blocked by a garbage can. The garbage can was removed during the inspection removing the partial obstruction. The lock underneath the kitchen sink does not function as a lock and can easily be removed leaving cleaning supplies accessible to persons in care. The outside BBQ did not have a cover on it and control knobs for the gas were accessible to persons in care.
