- Sanitizer was not prepared at the time of the inspection. A 200 ppm Quat solution was prepared onsite. Sanitizer is to be prepared at all times, and use for surfaces, equipment and utensils.

- When asked for sanitizer, staff provided multiple chemicals to the PHI. Ensure staff know when to use each specific chemical and that training is provided so that chemicals are not mixxed.
