      • As appeared in the photos sent by the owner / operator of this facility:QAC test strips are obtained by the owner / operator whereas CS20 requires AHP test strips for testing.Ensure AHP test strips are obtained to test CS-20 and maintain the log daily.

      • Some contaminated hair massage equipment were noted on carts beside bed in massage room.Staff took contaminated equipment for reprocessing.Ensure all reusable equipment and tools are properly disinfected after each use.

      • 1. Container used to submerge reusable scrapers is not sufficient as this container can only keep scarpers half submerged in CS-20. 2. Manufacturer instructions are not being followed, contact time is not tracked and equipment are kept longer in CS-20.3. CS-20 is expired on November 09, 2024.4. No test strips are available, neither High Level Disinfectant logs are completed.1. Ensure all reusable equipment are disinfected in a container which can keep all equipment fully submerged.2. Ensure labels are read, understood by each staff and manufacturing instructions are followed.3. Ensure a new CS-20 disinfectant is obtained and used within the expiry period.4. Ensure test strips are obtained for verifying the High-level disinfectant - CS-20 and HLD logs are completed daily. HLD - CS-20 solution can be reused for 14-days if it doesn't fail the test during this time period.
