      • 1. A bathroom cleaning chemical was stored beside the handwashing sink in the toddler room bathroom and was accessible to children.The cleaning chemical was relocated to an area inaccessible to children during the inspection.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Craft materials in the toddler room (ages 12 months to 3 years) were observed to have manufacturer age labels indicating “3+” and “5+.” The Primo Bingo dabbers are for ages 12 and older. These materials are not age appropriate for the children using the room.Review the craft box contents and remove or replace any materials that are not age appropriate for the toddler age group.

      • 1. During the diapering process in the toddler room, staff did not change gloves after removing a soiled diaper. The same gloves were then used to redress the child, touch the door surface, and handle the sanitizer bottle, creating a risk of cross-contamination.Onsite education was provided. The alternate program supervisor acknowledged that gloves should have been changed prior to redressing the child.

      • 1. The plastic couch in the Kinder room was observed to be in disrepair, resulting in surfaces that are not smooth and are difficult to effectively clean and sanitize.Repair or discard the couch to ensure the surface can be adequately cleaned and sanitized.

      • 1. Bleach sanitizer solutions used at the facility were labeled and tested at concentrations of 250 ppm and 500 ppm.For regular operations, bleach sanitizer must be maintained at 100 ppm. A higher concentration of 1000 ppm is required only during an outbreak situation.The bleach sanitizer solutions were changed to the appropriate concentration during the inspection.
